Three. Two were filled when I was around 10 years old. The third was fixed this past year (over 40 years later).
I have also had extensive dental work as a result of a car accident that "jarred" my jawbone and teeth, but no amount of preventative care or healthy oral hygiene habits would have prevented the need for that work. The cavities came because the teeth had to be covered with veneers, and eventually the veneers came loose, and bacteria got between the veneers and the teeth, and then boom - time for a bridge. But it took around 15 years from the time I got the veneers til I needed the bridge.
If you count the teeth that decayed or were otherwise compromised as a result of the car accident, it'd be 12.
Why are you taking a poll on this what is the purpose of this. Dose someone have to much time on there hand. It's like taking a poll on how many zits do you have in your face it's stupid.
