My PSA has been either slightly elevated or elevated from the time I was about 40 to 62. Regular checkups and biopsies over the years showed no cancer until age 62 when my PSA was 9.0. Did watchful waiting for about 6 months but PSA spiked to 15 then and decided to go ahead with RP. My Gleason was 3+4 = 7 and have one core with cancer at 2% involved.
I think PSA levels are VERY individual . . . some will run high all the time and just be BPH like mine did for years until the cancer showed on the last biopsy which still, I think, was pretty minimal.
