My wife and I refinanced our home about two weeks ago. We had been on a 30-year mortgage with a 6.75% APR, but were fortunate enough to land a 15-year at 4.5%.

Something just occurred to us over dinner. We weren't told (and didn't think to ask at closing) if we would be required to submit another Homestead Exemption form after the refinance.

Does anyone know? I wasn't able to find a definitive answer using Google. Thank you in advance.

No you don't..same property. You file on where you live as of Jan 1 and do not need to refile if nothing changes.
Homestead doesn't care about the mortgage.
