All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Over 30 live smallflies were found on the rear dish washing area ceiling. Remove all evidence of pest activity and clean up all areas of possible harborage. Clean and treat the floor drains and remove smallfly buildup.
The walls and ceilings shall be in good repair and easily cleaned. The tiles in the rear dishroom, mid point kitchen and next to the fryer are missing and need to be replaced.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Replace the sheild above the pizza prep area.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Repair the poultry cooler coil, it leaks.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Clean and maintain the floors and baseboards so they are smooth and sanitary from corner to corner.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOORS UNDER THE POT SINK HAVE BROKEN CORNERS, RESEAL THE BASEBOARDS. THE FLOOR UNDER THE HOT LINE IN FRONT ARE FULL OF BLACK GREASE BUILDUP, CLEAN THE FLOORS IN DETAIL.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE SPACE BETWEEN THE FRONT LINE GRILL AREA COOLERS IS FULL OF OLD FOOD, CLEAN, SANITIZE AND MAINTAIN IN SANITARY CONDITION. THE BREAD SHELVES IN THE WALK IN COOLER ARE MOLDY, CLEAN AND SANITIZE THE SHELVES. THE BEER DRAINAGE AREA IS DIRTY, CLAEN AND RESEAL THIS AREA.

MUST REPLACE BROKEN CEILING TILES AND WATER DAMAGED CEILING TILES IN STAFF RESTROOMS AND CUSTOMER RESTROOMS. MUST REPAIR HOLES ON WALLS AT HALLWAY LEADING TO REAR FIRE EXIT DOOR. MUST ALSO REPLACE TORN WALL PAPER AT SAME SAID AREA SO AS TO BE SMOOTH AND EASILY CLEANABLE.
MUST PROPERLY STORE WASH CLOTHS IN SANITIZING SOLUTION AT ALL TIMES.
MUST DETAIL CLEAN FLOORS UNDER DISH MACHINE AREA AND FRONT MAIN KITCHEN LINE.
MUST DETAIL CLEAN INTERIOR OF DESSERT COOLER AND FREEZER TO REMOVE FOOD SPALSHES. MUST REPAINT ALL RUSTY SHELF RACKS IN ALL WALK-IN COOLERS.MUST DETAIL CLEAN FRYERS TO REMOVE EXCESS GREASE BUILD-UP.
