All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED APPROX. 40 LIVE FRUIT FLIES BEHIND THE BOTH BAR AREAS AND SERVICE STATION HALLWAY NEXT TO THE PREP AREA. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REPLACE THE BROKEN DOOR GASKETS INSIDE OF THE SALAD PREP COOLER. MUST PROVIDE COVERS FOR THE ICE BINS BEHIND ALL BARS.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE BOTTOM SHELF OF THE PREP TABLE ACROSS FROM THE SALAD PREP COOLER AND INTERIOR OF THE ICE MACHINE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MAINTAIN THE FLOOR DRY BEHIND THE WINE BAR. DETAIL CLEAN THE FLOOR BEHIND THE BARS, DRY STORAGE AREAS AND BASEMENT BEHIND THE CHEST FREEZERS.
The walls and ceilings shall be in good repair and easily cleaned. REPLACE THE BROKEN AND MISSING WALL TILES IN THE PREP AREA. DETAIL CLEAN THE WALLS OF THE PREP AREA, DRY STORAGE ROOMS AND BEHIND THE MAIN BAR. REPLACE THE FRP BOARD BEHIND THE 3 COMPARTMENT SINK IN THE PREP AREA.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST INSTALL A SELF-CLOSING DOOR DEVICE AT THE EMPLOYEE RESTROOM. THE VENTILATION FANS IN THE MEN'S AND WOMEN;S RESTROOMS ARE NOT OPERATIONAL. MUST REPAIR.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE A THERMOMETER INSIDE OF ALL OF THE PREP COOLERS.

OBSERVED LIQUOR BOTTLES AND BOXES OF FOUNTAIN POP STORED OUTSIDE IN THE BEER GARDEN AREA. MANAGEMENT INSTRUCTED TO MOVE ALL ITEMS INSIDE OF THE PREMISES. SERIOUS VIOLATION 7-38-005A.

A separate and distinct offense shall be deemed to have been committed for each Serious violation that is not corrected upon re-inspection by the health authority. PREVIOUS SERIOUS VIOLATION FROM REPORT #277278 ON 7/12/10 NOT CORRECTED: 18/01 - OBSERVED OVER 100 LIVE FRUIT FLIES BEHIND BOTH BAR AREAS AND WAITRESS SERVICE STATION HALLWAY NEXT TO THE FOOD PREP AREA. OBSERVED FRUIT FLIES SETTLING ON BOTH BEER TAPS, CHAIRS, WALL AND MIRRORS AROUND THE FRONT BAR AREA. OBSERVED FRUIT FLIES INSIDE AND AROUND THE ICE BIN THAT WAS USED TO STORE BOTTLES OF WINE AND THE SODA GUNS. OBSERVED FRUIT FLIES AROUND THE ROLLING DISH RACK (WITH STANDING WATER INSIDE OF IT) IN THE WAITRESS SERVICE STATION AND ON THE REFRIGERATORS AND COFFEE GRINDER AND BOXES ON THE TABLE, WHICH IS NEXT TO THE FOOD PREP AREA. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE OR MINIMIZE THE PEST ACTIVITY. CRITICAL VIOLATION 7-42-090.

VENTILATION FOR THE PIZZA OVEN MUST BE PROPERLY CONNECTED.
