REATTACH VENTILATION FAN COVER TO VENT FAN ON CEILING OF WASHROOM. REMOVE DUST FROM VENT COVER.

OBSERVED FIFTY(RODENT)MOUSE DROPPINGS ON FLOOR BY REAR DOOR AND AT BOTTOM OF FREEZER THAT SITS BY REAR DOOR; INSTRUCTED TO CLEAN AREA AND HAVE PEST CONTROL COMPANY SERVICE ESTABLISHMENT PRIOR TO REINSPECTION. REAR SIDE EXIT DOOR NOT COMPLETELY RODENT PROOFED AT BOTTOM; SEAL FOR A TIGHTER FIT AND/OR REPLACE DOOR SWEEP AT BOTTOM OF SAME DOOR.
UPON ENTERING REAR KITCHEN FOR INSPECTION; I OBSERVED A CIGARETTE BUTT ON THE FLOOR. I ALSO OBSERVED TWO IN A CONTAINER OF OIL BY THE REAR DOOR, TWO INSIDE OF UTILITY SINK, TWO UNDER A REFRIGERATION UNIT, ONE BEHIND ANOTHER REFRIGERATION UNIT, TWO ON TOP OF CATCH BASIN ON FLOOR IN FRONT OF THREE COMP SINK AND AN EMPTY CIGARETTE BOX ON THE FLOOR BY THE REAR DOOR. INSTRUCTED TO REMOVE ALL BUTTS AND REFRAIN FROM SMOKING INSIDE OF THE ESTABLISHMENT.
