Observation: Walk-in door latches observed in a state of disrepair and damaged. (do not snap tight).
Correction: Repair the latches to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the latches, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: The white plastic cutting board(s) are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
- Physical Facilities in Good Repair
Observation: Ceiling tiles on the grill / hot food line need to be replaced with smooth / easy to clean tiles and are not maintained in good repair
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Physical Facilities - Cleaning Frequency and Restrictions
Observation: Tile cove molding needs cleaning all around where the mop hits.
Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

- Equipment - Good Repair and Proper Adjustment
Observation: One side of the pizza deli has a handle needing repair (screw missing) was observed in a state of disrepair and damaged.
Correction: Repair the handle to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the handle replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
- Handwashing Lavatories - Maintaining and Using Handwashing Lavatories
Observation: Handwashing facilities are not maintained. (soap dispenser is full..just needs the top attached to the back wall to keep it from flipping to the side).
Correction: Keep handwashing facilities maintained to encourage proper handwashing.

Observation: Storage room #3: push brooms, sweep brooms, equipment, etc. On the floor and clutter in the room.
Correction: Maintenance equipment supplies and tools such as brooms, mops, vacuum cleaners, and similar items must be stored in a neat and organized way.

Observation: The final rinse thermostat was observed in a state of disrepair and damaged. (dial would not move from 150 degrees. A sanitizer tape was sent through the machine on a plate and came ou with 180 degrees color change.
Correction: Repair the thermostat to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the thermostat or replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: The nonfood contact surface of several warming drawers where the slide rails had grease and soil built up.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

Observation: Observed methods are not being used to control pests (Pest Masters was notified by phone of the finding and was scheduled for the next day to correct this problem).
Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.

Observation: The nonfood contact surface of the cooks line fryer area equipmnet has accumulations of grime and debris. (grease)
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
- Refuse - Covering Receptacles
Observation: Observed filled refuse containers stored in the establishment containing food residue wastes without lids. (lids are bent and broken)
Correction: Cover all waste containers when not in continuous use. (Waste Management was called at this inspection to replace this dumpster).
- Physical Facilities in Good Repair
Observation: #1: Kitchen ceiling along the wall for the dry store room and the mop room needs to have the tiles painted (rust stains) and is not maintained in good repair.
#2: The ceiling lights in the dishroom have burned outbulbs.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
Ceiling light bulbs were replaced by maintenance before this inspection was completed. (CORRECTED)
