Observation: y
Correction: The person in charge failed to state the required temperatures and holding times for the potentially hazardous food items when (1) in refrigerated storage, (2) in hot holding, (3) being cooled and/or (4) being reheated 5) Cold Holding. OBSERVED THAT THERE WAS NO CERTIFIED FOOD MANAGER ON SITE AND THE STAFF PRESENT WAS UNABLE TO ANSWER QUESTIONS RETAINING TO COLD HOLDING, HOT HOLDING AND COOKING.
- Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (corrected on site)
Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required. OBSERVED THAT THE INSIDE OF THE ICE BINS HAD A LAYER OF PINKISH BLACK MOLD LIKE SUBSTANCE IN SOME AREAS.
Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold. DISCUSSED WITH MANAGER, ICE BINS WILL BE CLEANED AT CLOSE OF BUSINESS TODAY. RECOMMEND CLEANING ON A REGULAR BASIS.

Observation: There is no Certified Food Manager present at the beginning of the inspection. OBSERVED NO FOOD MANAGER WITH VALID FOOD MANAGER CARD WAS PRESENT AT THE FACILITY ALMOST TILL CONCLUSION OF THE INSPECTION.
Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. A MANAGER WITH VALID CERTFIED CARD ARRIVED APPROXIMATELY TOWARDS THE END OF THE INSPECTION. RECOMMEND THAT AN ADDITIONAL STAFF MEMBER BE TRAINED AND CERTIFIED AS FOOD MANAGER,
- Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F (corrected on site)
Observation: Water from the handwashing sink at BACK KITCHEN was measured at a temperature less than 100°F. OBSERVED THAT THE TEMPERATURE OF WATER AT THE HANDSINKS AT THE BACK KITCHEN AREA WERE 82F AND 85 F
Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet. THE WATER HEATER WAS ADJUSTED AND TEMPERATURES AT THE SINKS WERE 110F.

Observation: Observed the handsink at the BACK NEAR OFFICE IS being used TO STORE FOOD SLICER. ALSO REPAIR THE PAPER TOWEL DISPENSER.
Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. DISCUSSED WITH STAFF, SLICER WAS REMOVED FROM THE SINK.
