This visit was conducted in order to perform a risk factor inspection. The following issues require correction:
1. Turkey was observed cooling in tightly covered containers in the walk in cooler - Food containers in which food is being cooled shall be:
1) arranged in the equipment to provide maximum heat transfer through the container walls, and
2) the food containers shall be loosely covered or uncovered as long as protected from contamination to facilitate heat transfer from the surface of the food
.
2. Cans of pesticide/insecticide observed in chemical closet insecticide will be discarded, and facility has a contract with EcoLab, and receives regular pest control treatments.
3. PIC agreed to send copy by fax or email of FPM card to Lydia Zweimiller within two business days. Lydia.Zweimiller@vdh.virginia.gov

  • Critical: Pasteurized Eggs & Egg Products in Certain Recipes for HSP (corrected on site)
    Observation: Raw shell eggs are used in the preparation of the following ready-to-eat foods without subsequent cooking in the facility that serves a highly susceptible population: Scrambled Eggs are prepared with pooled non pasteurized shell eggs.
    Correction: In a food service establishment that serves a highly susceptible population, pasteurized eggs or egg products shall be substituted for raw eggs in the preparation of foods such as Caesar salad, hollandaise, or Bearnaise sauce, mayonnaise, meringue, eggnog, ice cream, and egg fortified beverages and recipes in which more than one egg is broken and the eggs are combined. Discussed with manager that use of pooled eggs are not permitted, and that pasteurized carton eggs will be used in place of the shell eggs.
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site)
    Observation: Observed that paper towels were inaccesible at 2F handwashing sink. Autromated dispenser was not functioning.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. Paper towels provided by manager on duty.

This visit was made to conduct a risk factor assessment. Good holding and cooking temperatures observed. Great hand washing. Keep up the great work!

08/19/2015Risk Factor
This visit was made to conduct a routine inspection 30 days after the change of ownership.
All noted violations on the pre-opening inspection report have been corrected. Great work.
There continues to be an issue with cockroaches on the HC level, but it is being managed by the pest control company and staff satisfactorily. Continue to check and change traps everyday as well as sealing food and cleaning to prevent contamination of food.

  • Nonfood-Contact Surface/Corrosion Resistant/Nonabsorbent
    Observation: The nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth: (The paint on the racks in the walk-in is chipping and flaking off)
    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. (clean and re-paint, or replace racks in the walk-in)
  • Ventilation Hood Systems / Adequacy
    Observation: Grease/Condensation is noted to be accumulating on the walls/ceiling at the dish machine in the kitchen. This appears to be due to insufficient ventilation of the hood system.
    Correction: Ventilation hood systems and devices shall operate properly and be sufficient in number and capacity to prevent grease or condensation from collecting on walls and ceilings.
  • Critical: Backflow Prevention, Air Gap / When Required
    Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin. (Hose at mop sink was resting below flood rim level.)
    Correction: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by Law by providing an air gap as specified under 5-202.13. (Person in charge agreed to cut the hose)
  • Physical Facilities Good Repair
    Observation: Observed that the caulking around the dish mahcine (moldy and broken), ceiling tiles around the dish machine hood (metal frame rusty), walk-in floor (uneven and gap not sealed where metal flashing meets), floor tiles and coving tiles in the kitchen (some are missing or broken), and the floor/wall juncture in the HC level kitchen (wall is damaged, coving is missing/broken) are not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Controlling Pests / Methods to Use When Pests Are Found
    Observation: Methods being used to control pests are not adequate. (Observed trapped and live roaches in the HC level kitchen)
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence. (In addition to pest control services, staff must clean dead insects, keep the kitchen areas clean (especially the floor), and eliminate harborage conditions (damaged floor and wall areas)).
