    Observation: Dispensing utensils improperly stored between uses in that ice scoop observed stored on top of ice machine and not in the adjacent plastic pitcher placed their to store the ice scoop..
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: Plumbing connections under the handwash sink opposite the food manager's office are leaking. The sink was observed with a bucket underneath the hot and cold water shutoff valves and the sink was shut off.
    Correction: Plumbing systems and components shall be maintained in good repair.

    Observation: The 4 outdoor refuse containers is stored on absorbent material.
    Correction: All refuse containers are to be stored on smooth, nonabsorbent material that is sloped to drain.

    Observation: Toilet was not accessible to employees during all hours of operation due to the employee rest-room being out of order over the previous 5 years due to chronic plumbing issues.
    Correction: Toilet must be conveniently located and accessible to employee during all hours of operation to encourage employee use of appropriate facilities for disposing of human wastes as needed followed by the washing of hands.

    Observation: Recently repaired Arctic Aire Glass Door Refrigeration unit was observed at 48 °F and is not sufficiently capable in temperature to meet the food storage demands of the establishment.
    Correction: Equipment for holding cold food, shall be sufficient and capable to maintain food items at 41 or below. Improper food storage temperatures are a major contributing factor to foodborne illness.

    Observation: Plumbing connections under the 3-compartment sink are leaking.
    Correction: Plumbing systems and components shall be maintained in good repair.

    Observation: Hand wash sink opposite food managers office is in poor repair in that hand wash sink was observed partially detached from wall leaning to one side.
    Correction: Repair and maintain all plumbing components and fixtures.

    Observation: Toilet was not accessible to employees during all hours of operation in that food employee toilet is out of order and has been for the past 5 years.
    Correction: Toilet must be conveniently located and accessible to employee during all hours of operation to encourage employee use of appropriate facilities for disposing of human wastes as needed followed by the washing of hands.
