The purpose of today's visit is to conduct a routine inspection in conjunction with a Risk Factor Survey as part of FDA Retail Standard 9.
Please make sure the water at both the handsinks (restroom) and kitchen is at minimum 100F.

  • Critical: Package Integrity & No Dented Cans/ Specifications for Receiving (corrected on site)
    Observation: The following food items from damaged packaging were found offered for sale or service: 3 dented cans for sliced peaches.
    Correction: Food packages such as canned goods shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Return all dented and damaged canned goods to your food supplier for a credit. Note: Separeated the cans.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
    Observation: Water from the handwashing sink at employee restroom at 67F and kitchen handsink 95F was measured at a temperature less than 100°F.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.

Note: Chicken and cheese on salad could be cooling from delivery time. Discussed with the employee to check the potentially hazardous foods on salad (check chicken instead of lettuce).
Manager is knowledgeable and will monitor the refrigerators once during the service hours. Reviewed temperature logs.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: yogurt 50F, chicken on salad 63F--Delfield 2DR UR outside
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. Note: Put items in back milk cooler. Note: The refrigerator is in use, could have gone through a defrost cycle.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
    Observation: Water from the handwashing sink at 98F was measured at a temperature less than 100°F.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet. Note: The faucet turns on automatically (sensor). It takes a while for the water to get hot.

This is pre-opening inspection for the renovation. Permission is granted for stocking of food and utensils.
Approval by Health Department is granted.
*Please change employee restroom ceiling tiles to cleanable tiles. Please send me a picture of the correction.

08/24/2015Pre-Opening
Discussed with the manager about monitoring and recording cooling temperatures. The manager is knowledgeable and is proactive in monitoring things. She mentioned that they saw some mouse activity due to construction and has already contacted the right department to monitor and control the infestation.
Reviewed calibration of thermometer and temperature logs. Recommend using the thermometers inside the refrigerator/freezer to record the ambient temperatures.
The manager does not know if there is a grease trap outside the facility. None observed indoors.

  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (repeated violation)
    Observation: Outside refuse container was uncovered with damaged lids.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment. Note: Replace the lids for the dumpster.
  • Refuse / Drain Plug (repeated violation)
    Observation: Both refuse containers used to store refuse/recyclables has no drain plug.
    Correction: Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place. Note: Provide new drain plug for the dumpsters.
  • Physical Facilities Good Repair
    Observation: Observed that couple ceiling tiles in the dry storage room are damaged and have a small hole that could be an entry point for pests.
    Correction: Replace the ceiling tiles.
01/15/2015Routine
The purpose of this visit was to conduct a risk factor assessment. Quaternary ammonium sanitizer 3 vat sink/ wiping cloth bucket: 300/300 ppm. The following good retail practices were discussed during the inspection: 1) Documentation on exhaust hood over cookline states hood was due for cleaning in August of 2014.

  • Critical: Certified Food Manager/Presence Required
    Observation: There is no Certified Food Manager present in the establishment.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
