The purpose of today's visit is to investigate a complaint received by the Fairfax County Health Department on March 13, 2014. The complainant alleges consuming approximately 2/3 thirds of the hot chocolate drink to discovered a cockroach in the complainant mouth.
EHS discussed the complaint with the CFM and was aware of the complaint. The food facility immediately scheduled pest control service the same day the complainant reported the issue to the facility. EHS observed pest control service reports on 3/13/14, 3/14/14, 3/17/14, and 3/18/14. At the time of the visit, EHS observed no pest activity during inspection. Complaint is not confirmed at the time of the inspection.

The purpose of today's visit is to conduct a routine inspection in conjunction to a complaint received to the Fairfax County Health department on March 13, 2014.
  • Permit to Operate Required, Must be Posted, Must be Valid
    Observation: The establishment has not posted the valid permit to operate in a conspicuous place in the public view.
    Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.

The purpose of today's visit was to conduct a complaint investigation. A complaint was received by the Health Department on September 9, 2013. The complainant visited the facility on Saturday, September 7. As a part of the complainant's order, a cup of ice water was requested. The complainant alleges that inside the cup of ice water a bug was observed. EHS discussed the complaint with the manager, who was aware of the complaint. Manager showed EHS the clear plastic cup with clear top that is used to provide free cups of ice water. Facility receives monthly contracted pest control service. The last service was received on September 4, 2013. EHS observed facility clean, organized and observed no pest activity during inspection. Complaint cannot be confirmed.
