Observation: The cardboard shelf unit in kitchen is not designed and constructed to be durable. This unit is absorbant, nondurable, and can not be properly cleaned.
Correction: Remove this item from the facility and replace with an approved unit as needed. Equipment and utensils must be capable of maintaining their original characteristics in order to be easily cleaned and to prevent contamination of food.

Observation: Employee failed to state the required temperatures and holding times for the following conditions of potentially hazardous food: (1) Refrigerated storage, (2) Hot holding, (3) Cooking, and (4) Reheating.
Correction: Ensure employees are knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The person in charge (PIC) should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.
- Equipment - Cutting Surfaces
Observation: The cutting board on the Turbo Air make table refrigerator is heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
- Good Repair and Calibration of Thermometers (corrected on site)
Observation: Observed food thermometer is not calibrated to ensure accuracy.
Correction: Calibrate food thermometer according to manufacturer's specification as necessary to ensure accuracy. Dial thermometers can be calibrated with ice water. Place stem in the ice water, use a small wrench to turn the nut on the underside of the dial until it reads 32 deg. F. You also need to purchase at least 2 metal stem food probe thermometers with a small diarmeter probe.
- Non-Food Contact Surfaces
Observation: The nonfood contact surface of the ventilation hood vents has accumulations of grime and debris.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil greaseesidues.

Observed good handwashing and glove use practices. Time control being used for the hot held items in the hot holding display case. Written logs kept daily. No potentially hazardous foods held in this unit over 4 hours. Foods not saved or re-used after 4 hours. Sanitizer in 3 compartment sink 200 parts per million quaternary ammonium compound. Ok. Establishment is still on pump and haul for disposal of sewage. Store manager informed me that they had changed pumpers. I did not recognize the name of their pumper as one that is permitted in Virginia. I advised her that her pump and haul permit requires that their pumper be licensed to pump sewage in Virginia.
