Observation: Some food on display is not protected from contamination. The bowls of peanut butter and jelly and whipped butter have neither lids on the bowls, nor sneeze guard panels over them.
Correction: Protect food on display by the use of packaging

Observation: The sliced bread in bags at the buffet line is not provided with dispensing utensils.
Correction: Provide separate dispensing utensils for each food item at the salad bar or buffet line to prevent cross contamination between foods or the possibility of bare-hand contact (by consumers reaching into the bags with their hands) with RTE food intended for other consumers.

Observation: One bulk milk container tube was observed not cut on the diagonal (a 45 degree angle).
Correction: A bulk milk container tube shall be cut on the diagonal to allow drainage of the tube, and shall be positioned to extend no more than 1 inch from the dispensing head to minimize potential for contamination.
- Non-Food Contact Surfaces
Observation: The nonfood contact surfaces of the door handles (on the reach-in coolers, on the hot-holding cabinet, etc.) have accumulations of grime and debris.
Correction: Clean these surfaces at a frequency necessary to preclude accumulation of soil residues that become encrusted, or that attract insects and rodents.
- Floors, Walls, and Ceilings - Cleanability
Observation: Floor or floor covering -- around the floor drain situated under the prep counter across from the hot line -- is not smooth and easily cleanable. (a) White reinforced tape, around the perimeter of the drain, presents a cleaning hazard: the tape is coming loose and rumpled, and shows accumulations of dirt and grime. (b) a trough-shaped piece of metal, placed across the drain to support prep table legs, also is a cleaning challenge (accumulation of crumbs and other debris in an area that cannot easily be cleaned).
Correction: Repair or replace floor or floor covering to make it smooth and easily cleanable. Devise alternate method to support the table legs

Observation: Plumbing connections under the "wash" vat of the 3-vat sink are leaking.
Correction: Plumbing systems and components shall be maintained in good repair. PIC stated that the maintenance department had already been notified of the need for repair, which was awaiting acquisition of a part. The facility staff is requested to notify the health department when the repair has been made.
