Critical: Hands - When to Wash* (corrected on site) Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils or after engaging in any activity which may have contaminated his/her hands. EHS observed employee changed gloves and did not wash before putting on new gloves. EHS educated employee as to when to wash hands. Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task. Jewelry - Prohibition (corrected on site) Observation: Employees wearing jewelry on their arms and hands while preparing food. EHS observed two employees wearing bracelets and watch. Employees removed. Correction: Ensure employees are not wearing jewelry on their arms and hands while preparing food. This section does not apply to a plain ring such as a wedding band. Food Storage Containers - Identified with Common Name of Food (corrected on site) Observation: Unlabeled food containers. EHS observed food container (flour) not labeled. Correction: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning. Nonfood Contact Surfaces (corrected on site) Observation: The nonfood contact surface of the shelves is not designed or constructed to be easily cleanable. EHS observed shelves lined with soiled absorbant towels (not easily cleanable). EHS also observed duct tape attached to hood system. PIC removed. Correction: Alter or replace this item to provide a surface that is free of unnecessary ledges, projections, and crevices. Temperature Measuring Devices Observation: There was no temperature measuring device located in the small reach in refrigerator and chest freezer. Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature. Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils Observation: Observed accumulations of encrusted grease deposits or other soil on the following food contact surfaces: Ice machine. EHS observed black mold inside ice machine. Correction: Clean and sanitize these surfaces for food contact. Critical: Backflow Prevention Device, When Required* (corrected on site) Observation: Backflow or backsiphonage prevention device not available on threaded faucet as required by law. EHS observed a hose attached to mop sink without a backflow preventer. EHS educated PIC about attaching hose. Correction: Provide a backflow or backsiphonage prevention device on all threaded faucets. Plumbing System Maintained in Good Repair Observation: Hand sink and prep sink in poor repair. EHS observed leaks from hand sink drain line, and also a leak from prep sink faucet. PIC made service call. Correction: Repair and maintain all plumbing components ans fixtures. Mops - Drying Mops (corrected on site) Observation: Mops not hung up to air dry. EHS observed mop lying on floor of mop sink. PIC hung mops. Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use. Critical: Toxics - Storage of Toxic Containers* (corrected on site) Observation: Containers of cleaning agents not stored separately from insecticides or rodenticides. EHS observed insecticides stored with cleaners. PIC removed. Correction: Store detergents, sanitizers, related cleaning or drying agents and caustics, acids, polishes and other chemicals separately from insecticides and rodenticides.	11/24/2015	Routine

Date marking is in place but watch the correct dating and that foods are discarded after 7 days. Proper hair restraints and glove use was observed. Manager acknowledged importance and will add consumer advisory where needed. - Discussed date-labeling with manager. - Discussed proper food storage in cold-holding units to prevent cross contamination. - Dry food storage area observed neat and organized. - Proper hand-washing and glove-use observed during inspection. - Quaternary ammonium test strips in place near three compartment sink. - Reviewed employee health with manager. Manager very knowledgeable in employee health. - Reviewed wiping bucket procedure for dining area vs. kitchen. Manager acknowledged proper process. - Great job!
