Four lights under hood system at the cook line has been corrected. Two lights under th e hood system at the pizza oven still needs replaced.new lights are at the facility and PIC states he will replace bulbs when oven is cooled down at the end of the day.

    Observation: 4 out under cook line hood system. and 2 lights out under pizza hood and is not maintained in good repair. Proper lighting affects the light intensity for proper safe working conditions.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

EHS observed dust grease build up under the pizza hood system. Clean area so dust and debris are not blown on food and food surface items. Ensure all food handler cards a re current and available at the facility. Keep up all date marking and repair light as required. Understanding food safety is very important ensure all workers are familiar with food temperatures and employee health.

    Observation: Employees are not properly trained in food safety as it relates to their assigned duties. No records at facility of current food handler cards. Ensure records are current and maintained at the facility for inspection. Food handler cards are required by city ordinance sec 49 and violation is a class 1 misdemeanor.
    Correction: Train all employees in food safety as it relates to their assigned duties.
  • Critical: Employee Health* (corrected on site) (repeated violation)
    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion. Employees or applicants are not aware of the reporting procedures concerning information about their health and activities if they are suspected of causing, or being exposed to a confirmed disease outbreak caused by Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus. Personnel could not answer signs and symptoms and disease questions. EHS went over them and provided flyers for future use.
    Correction: Have the license holder require employees or applicants report to the person in charge if they are experiencing the following symptoms or conditions: (1) diarrhea, fever, vomiting, jaundice, or sore throat with fever. (2) A lesion containing pus such as a boil or infected wound that is open or draining and is on the hands or wrists, unless an impermeable cover such as a finger cot or stall protects the lesion and a single-use glove is worn over by an impermeable cover or on other parts of the body, unless the lesions covered by a dry durable, tight-fitting bandage. Have the license holder require employees or applicants report to the person in charge if they meet one or more of the following high-risk conditions: Is suspected of causing, or being exposed to, a confirmed disease outbreak caused by S. Typhi, Shigella, Escherichia coli O157:H7, Hepatitis A virus or norovirus

    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

    Observation: The prepared ready-to-eat (RTE) mozz cheese in the refrigeration unit is not properly dated for disposition. Cheese is sliced at the facility. Corrected during inspection.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: There was no temperature measuring device located in the first prep unit.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

    Correction: Train all employees in food safety as it relates to their assigned duties. Food handler cards need up dated no later than 30 days of this inspection. Ensure cards or copies are kept at facility for inspection at all times.

    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.

    Correction: Mark a "consume by" date sliced RTE foods at the time the original container is opened. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days.   • Equipment - Good Repair and Proper Adjustment
    Observation: lights under hood vent are missing was observed in a state of disrepair
    Correction: Repair the lights to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the ligths, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Mops - Drying Mops
    Observation: Mops not hung up to air dry. Observed mop drying in mop bucket. Ensure mop is hanging with mop head downward.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: Dispensing utensils improperly stored between uses (dry goods).

    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: Lights over the ventilation hood are out.
    Correction: Maintain all lighting in proper working order.
