    Correction: Protect lamp against breakage by a shield surrounding and extending beyond the light bulb.Correct burned out issues with new bulbs.

Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: 1 Door True Freezer/Next to grill stations, was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.(Condensate/Ice water leaking on interior area and door gasket area.(Ice Buildup on bottom shelve and on food product).
    Correction: Repair the freezer/equipment to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the issue/freezer , replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: The prepared ready-to-eat (RTE) food in the 2 door True refrigeration unit is not properly dated for disposition.(Most food tossed out within 3-4 days.according to CFM/owner.Discussed this observation with Management/owner/CFM.
    Correction: Mark the name and ""consume by"" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.Observation corrected on site.

Maintain temperature logs daily on all refrigeration units/am-opening /pm-closing.Last IPM treatment accomplished on 2-26-2013.(Pest Control).No activity noted.Permit expires 12/2013.Permit and CFM certificate posted.Recommend that establishment certify a 2nd CFM for restaurant for backup during operational hours.No violations noted for outside trash area and restrooms. Facility is clean and well organized. Note:Make sure serving utensils stay clean and stored in food product service container during operational hours.(Discussed with owner/Manager).
