Observation: Mop not hung up to air dry.
Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

If unable to repair the hood system, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

- Warewashing Machines, Flow Pressure Device
Observation: The pressure gauge for the high temperature final rinse warewash machine is improperly installed. Gauge not measuring pressure, PIC called Ecolab to get them to come back out. New dishwasher installed about a week ago.
Correction: The pressure gauge for the high temperature final rinse warewash machine is to be installed upstream from the hot water sanitizing rinse control valve. The device must be mounted in a .25 inch Iron Pipe Size valve. Relocate the gauge.
- Physical Facilities in Good Repair
Observation: Wall behind ice machine observed with small hole in drywall, thus not maintained in good repair
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Critical: Pests - Controlling Pests* (corrected on site)
Observation: Methods being used to control pests are not completely effective. Insects observed under and around dishwasher. Facility is repairing wall and adding FRP but the FRP is not sealed to drywall yet, thus allowing insects a place to hide. Ecolab was called during inspection to come out and treat for insects and maintenance technician was shown the problem areas and what needs to be done to remove harborage conditions.
Correction: When pests are found use methods to such as trapping devices or other means of pest control. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.

Inspection done with Marcia Snyder for Standardization. Dishwasher gauges were showing temperatures below those specified on placard but TempRite sticker used and proper color change observed. Maintenance was brought it during inspection to be shown problem areas and issues that needed attention. New kitchen manager interviewed during inspection and left information for Certified Food Manager's class in June. PIC stated she would let Ecolab know that dishwasher needs to be looked at. WIll follow-up with informal visit next week to check dishwasher.

Observation: Hood system was observed with filters not correctly placed in brackets, thus not able to drain grease properly and in a state of disrepair and damaged.
Correction: Repair the hood system filters to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the filters, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of mold and debris: interior of ice machine and back panel of convection oven.
Correction: Maintain nonfood-contact surfaces of equipment clean.

Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin. Observed eye wash station without a backflow preventer on hose.
Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.

Observation: Standing dirty water was observed adjacent to the refuse container outside the facility, possibly dumped mop water.
Correction: The refuse container storage area and grounds adjacent to the container are to be maintained clean and sanitary.

Observation: There is no covered refuse container for the disposal of feminine napkins in the rest room stall.
Correction: Install a covered refuse container for the disposal of feminine napkins in the ladies room stall.

Observation: Less than 20 foot candles of light was noted in the dish room.
Correction: Provide at least 20 foot candles at a distance of 30 inches from the floor in consumer self-service areas, inside equipment, warewashing, equipment storage, toilet rooms

Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Had to run dishwasher 4 times to get proper sanitizing temperature. Brenda is going to get Ecolab to check dishwasher when they come out to check triple sink sanitizer.

Observation: When tested, no sanitizer or a low concentration of chemical sanitizer was found in the wiping cloth bucket. Observed at 100ppm but could not get 200ppm. PIC called Ecolab to come out and adjust tomorrow Instructed PIC to change buckets every few hours until then.
Correction: Provide quaternary ammonium at proper concentration of 100-200ppm and immerse or expose food contact surfaces to sanitizing solution for adequate time.
