    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: MILK 45F @ #5.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. MOVED BELOW (ON TOP FOR < 30 MIN.) & CFM SAID THEY WILL KEEP BELOW FROM NOW ON
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site) (repeated violation)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 2 of 3 wet washcloth buckets with a concentration of 0.0 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.
  • Refuse / Outdoor Containers /Capacity (repeated violation)
    Observation: The quantity of refuse exceeded the storage capacity of the exterior storage enclosure.
    About 7 bags of trash including french frys and other products sold at Elevation Burger, around dumpster.

    Correction: An outside storage area and enclosure and receptacles shall be of sufficient capacity to hold refuse, recyclables, and returnables that accumulate. Again shown to CFM.

    Observation: The premises is not being routinely inspected for the presence of insects, rodents and pests. Active rat type holes behind dumspter.
    Correction: Insects, rodents and pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Routinely inspect the premises to control the presence of insects, rodents and pests on the premises. Shown to CFM.

    Observation: Trash was observed adjacent to the refuse container outside the facility. 5 TRASH FILLED BAGS BEHIND DUMPSTERS IN COMPOUND.
    Correction: A storage area and enclosure for refuse, recyclables, and returnables shall be maintained free of unnecessary items and kept clean. CONTACT LAND LORD AND OTHER DUMPSTER USERS AND ALERT THEM OF PROBLEM.

    Observation: The premises is not being routinely inspected for the presence of insects, rodents and pests. ACTIVE RAT TYPE HOLES BEHIND DUMPSTER COMPOUND.

Routinely inspect the premises to control the presence of insects, rodents and pests on the premises. CONTACT EXTERMINATOR AND FILL IN RAT HOLES UNTIL NO LONGER OPENED.

    Correction: A complete employee health policy must have the following elements:
