Water has been shut off due to plumbing problems
and restroom has been removed.
Water is available at PIC's residence behind building.
Sanitizer pads, bleach and disposable gloves were available.
Disposable single serve items are being used.
2 compartment sink was present, but no water under pressure
and could be used for handwashing.
PIC says he has a plumber scheduled to repair the sink and told
me that it will be repaired by April or May.
PIC says he will contact me when repair is made.

  • Water Pressure
    Observation: Hot / Cold water under insufficient or inadequate pressure was found to serve the following equipment: 2 compartment sink.
    Correction: Inadequate water pressure could result in improper handwashing or equipment operation. Restore water under sufficient and adequate pressure to the 2 compartment sink immediately.
  • Critical: Handwashing Lavatory*
    Observation: There are not an adequate number of handwash lavatories on site. (See 12 VAC5-421-2280)
    Correction: Install additional lavatories to increase the employee's opportunity to wash hands routinely. Prior to installing additional plumbing fixtures the permit holder must apply and gain approval through the necessary regulatory agencies.

    Observation: There are not an adequate number of toilets required by law
    Correction: Install additional toilets to be in compliance with law. Prior to installing additional plumbing fixtures the permit holder must apply and gain approval through the necessary regulatory agencies.

Violation and inspection report were discussed with the PIC.
Hamburgers and chicken nuggets were cooked from frozen store bought (commercially processed)
Chicken salad and pork BBQ were commercially prepared.
No seafood.
A reminder: All foods must be prepared in restaurant.
No bare hand contact with any ready-to-eat foods.

  • Critical: Hands - Preventing Contamination from Hands* (corrected on site)
    Observation: Employees observed handling ready-to-eat (RTE) food with their bare hands.
    Correction: Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands.
  • Kitchenware and Tableware
    Observation: Single-service or single-use article intended for food- or lip- contact not furnished in original wrapper intact or from and approved dispenser.
    Correction: Dispense single-service and single-use articles intended for food- or lip-contact in orginal intact wrapper or from an approved dispenser
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: Soap was not provided at the hand washing lavatory in the kitchen.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
  • Hand Drying Provision (corrected on site)
    Observation: No disposable towels were provided at the hand washing lavatory in the kitchen
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials
  • Critical: Toxics - Storage of Toxic Containers* (corrected on site)
    Observation: Containers of cleaning chemicals are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items.
    Correction: Containers of cleaning chemicals must be located in an area that is not above food, equipment, utensils, linens or single service items.
