- Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
Correction: A complete employee health policy must have the following elements:
1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
- Storage of Clean Equipment, Utensils & Linens to Protect from Contamination (repeated violation)
Observation: Cleaned food containers and utensils are stored on a shelf immediately adjacent to a handwashing sink where they are subject to contamination.
Correction: Relocate clean utensils for storage away from handsink or install a splash guard.
- Fairfax City Code/Certified Food Manager/Certification Process
Observation: The Person in Charge (PIC) has proof of successful completion of an approved Certified Food Manager (CFM) exam but does not possess the Northern Virginia Certified Food Manager certificate (photo card) issued by ORS Interactive, Inc.
Correction: Obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to my attention at (703) 653-9448.

Observation: Observed that the shelf above the three vat sink used to store clean equipment is visibly soiled with mold and debris.
Correction: Clean this shelf, and maintain it clean.

The purpose of this visit was to perform a routine inspection.
Please send photograph or repair invoice indicating a proper air gap at the three vat sink to the EHS no later than 9 February 2015. Please send copy of Certified Food Manager's Photo Identification card to the EHS no later than 9 February 2015. Please remember that a certified manager must be present at all times of operations to oversee food safety procedures.
- Wiping Cloths/Use Limitation/Wet Towel Storage (repeated violation)
Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.
- Equip/Utensils/Linens/Single-Service/Storage, Location
Observation: Observed that clean pizza boxes were stored in an manner that exposed the item(s) to contamination.
Correction: Store clean equipment, utensils, and linens in a clean, dry location where they are not exposed to splash, dust, or other contamination and at least 6 inches above the floor. EHS ADVISED EMPLOYEES NOT TO STORE CAR TOPPERS ON UNWRAPPED PIZZA BOXES IN STORAGE.
- Certified Food Manager/Certificate Process
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action. EHS PROVIDED HANDOUT EXPLAINING HOW TO OBTAIN CFM PHOTO iDENTIFICATION.
- Critical: Air Gap Design Standard, Not Less than 1'
Observation: The air gap between the water drain of the three vat sink and the floor drain is less than one inch.
Correction: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch. PLEASE SEND REPAIR INVOICE OR PHOTO OF REPAIR TO EHS NO LATER THAN 9 FEBRUARY 2015.

Observation: The trash can cover was removed from the trash can in the ladies restroom.
Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
