The purpose of this visit was a follow up inspection to verify the cold holding capability of the Randell 4 door prep refrigerator. Presently it is holding PHF/TCS food at 41F or below.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Randell 4 door prep refrigerator 44F
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed/email to the food safety section at (703) 653-9448.
  • Critical: Backflow Prevention, Device / When Required
    Observation: Observed the lack of an approved backflow prevention device on the water supply system at the mop sink.
    Correction: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by Law by installing an approved backflow prevention device as specified under 5-202.14. INSTALL A BACKFLOW PREVENTION DEVICE AT THE MOPSINK
  • Critical: Sewage System / Operation and Maintenance
    Observation: Observed the grease trap lid has been permanently cemented to the grease trap, not permitting the grease trap to be cleaned.
    Correction: Repair the grease trap enabling the lid to be easily removed for regular cleaning.
  • Toilet Rooms, Enclosed / Self-Closing Door
    Observation: Toilet room door is not provided with a self-closing door.
    Correction: Provide a self-closing hinge or closing mechanism for the toilet room door.

The purpose of this visit was to conduct a routine inspection.
A follow up to assess the cold holding capability of the Randell 4 door prep refrigerator will be conducted on or about August 25, 2015.
Please email me a copy of your photo CFM card within a month. Additionally, please repair the grease trap, install a backflow prevention device and email me pictures of the corrections by September 4, 2015.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device:Observed penne pasta at 44F, sausage at 43F, shredded mozzarella at 43F, diced tomatoes at 46F, chicken at 43F
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.MOVED ALL PHF/TCS FOOD TO THE WALK IN TO COOL TO 41F OR BELOW.
  • Equipment / Good Repair / Operation

  • Storage of Clean Equipment, Utensils & Linens to Protect from Contamination (corrected on site)
    Observation: Observed that clean take out boxes were stored in an manner that exposed directly adjacent to a handsink and were subject to spray from handwashing.
    Correction: Store clean equipment, utensils, and linens in a clean, dry location where they are not exposed to splash, dust, or other contamination and at least 6 inches above the floor.MOVED BOXES AWAY FROM THE HANDSINK
  • Modifications to FDA Food Code/Section 6-402.11, Toilet Rooms Required with Seating, Not Accessed through Food areas, Accessible at all hours of operation (corrected on site)
    Observation: Observed no public restrooms in the food establishment and a counter with two seats.
    Correction: Public toilet rooms shall be provided for consumers whenever seating for consumers is provided on the premises. Consumers shall not be permitted to gain access to toilet rooms by passing through a kitchen, food preparation, food storage, or utensil washing area. Toilet rooms shall be conveniently located and accessible to employees during all hours of operation. Toilet rooms must be entirely separate and apart from any rooms or areas used for utensil washing or the manufacture, storage, and handling of food products. Public and employee toilet facilities shall be installed according to and in the number required by the Virginia Uniform Statewide Building Code. REMOVE SEATS FROM THE COUNTER AREA
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc. OBSERVED MANAGER HAS SERVESAFE CARD BUT DOES NOT POSSESS CFM CARD FROM ORS INTERACTIVE.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the FLOOR TILES, CAULKING AT THE HANDSINK AND THE COVEBOARDS NEAR THE GENERAL SINK AREA ARE MISSING AND OR IN DISREPAIR AND NEED TO BE REPAIRED..
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. SPOKE TO MANAGER AND ASKED THAT THE LISTED ITEMS BE REPAIRED.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Refuse Container with Cover/ Women's & Unisex Toilet Room
    Observation: There is no covered refuse container for the disposal of feminine napkins in the unisex room stall.
    Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
  • Physical Facilities Good Repair
    Observation: Observed that the wall - 3 VAT sink juntion has accumulations of dirt, grime and caulking has not been done preoperly. This area is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
