KDA_4_20211A2 - --On the East Wire Storage Racks, there were 2 plastic clear containers (stored as clean) with deep cracks in the food contact surfaces. Corrected on Site - Voluntary Discarded.
KDA_4_60111C - --There are 3 osculating fans in the kitchen are with dust build up on the fan blade covers. There is dust build up on the fan blade covers in both Walk In Coolers.
KDA_4_70211 - --While the Dish Washer was running dishes through the Warewashing Machine, the inspector’s thermometer read 131.7F. Corrected on Site - All dishes that were washed during this time were set aside until the Maintenance Personnel was able to fix the Warewashing Machine. The temperature was taken again and was at 166.8F. Note: The inspector's thermometer was calibrated on 10/13/15.

KDA_6_501111A - --There are approximately 10 flies flying around the kitchen area landing on food prep tables, food contact surfaces, and food shelves. One fly was also seen landing on a fully cooked RTE (Ready to Eat) cinnamon roll. Note: The cinnamon roll was voluntary discarded. There are no fly strips in the facility or fly lights seen in the facility.
KDA_7_10211 - --On the shelf underneath the table mounted can opener, there was a black bucket with clear liquid with white wiping cloths with no label. According to Person In Charge, the clear liquid is Chlorine Sanitizer. Corrected on Site - Labeled.
