Purpose of site visit: Evaluate facility for foodservice permit. Correct the following items: 1) Provide cart for dirty food equipment, tableware, & utensil storage for 3-vat sink. 2) Install large shelving unit for storage of clean food equipment, tableware & utensil in kitchen. 3) Install soap dispenser at kitchen handsink. 4) Install chemicals for dishwasher. 5) Post handwashing signage for kitchen handsink. 6) Install an approved splashguard behind & at the corner wall of the dishwasher. 7) Seal back of 3-vat sink at wall partition. 8) Install 100 watt bulb or fluorescent lamp equivalent in right exhaust hood light fixture. 9) Install filters in exhaust hood. 10) Remove shipping plastic from fryer & 3-vat sink. 11) Provide a sanitizer test kit for the 3-vat sink & dishwasher. 12) Install a back-flow prevention valves on the mop sink faucet & on the mop sink water line to the chemical mixer / dispenser.
