| The purpose of this visit is to conduct a first routine inspection. Employee Health Policy in place. Sanitizer observed within acceptable limits. Time as a Public Health Control (TPHC) information was provided to the manager in the event that the manager chooses to use for the par cooked french fries and raw chicken wings stored on ice, as well as wing sauces, which are in need of product assessment for shelf stability if this facility chooses to maintain these sauces at room temperature. 1. EHS provided information regarding obtaining your Certified Food Manager Photo Identification card. 2. Manager agreed to provide product assessment for the wing sauces within 30 days, to the health department as, per manager, the wing sauces are shelf stable and do not require temperature control. Meanwhile, maintain the wing sauaces at 41f or below until approval from the health department. I have provided you with a copy of my business card, so that you can email a copy of your Photo Certified Food Manager card and also provide product assessment for the wing sauces.

| The purpose of this visit is to conduct a first routine inspection. Manager and employees were very knowledgeable in a variety of area concerning food safety. EHS provided extra handwashing signs and other food safety information to use for training new employees. No critical violations observed during today's visit. Thank you. Manager agreed to provide product assessment for shelf stability for all of the wing sauces within 30 days.
