Due to the age of the building, the facility does not comply with current code requirements. In the event of a change in operator or remodeling, the facility must be brought up to the current code requirements. This may include the addition of one or more handsinks, improved lighting, repair of structural deficiencies, and repair or replacement of damaged equipment, and proper installation of laundry facilities.

  • Refuse - Covering Receptacles (repeated violation)
    Observation: Dumpster or outside refuse container was open.
    Correction: Cover all waste containers when not in continuous use.
  • Outer Openings - Protected
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. Screen door has a large gap at the bottom, of the right side of the door.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: The following physical structures is not maintained in good repair:
    >> Hole at wall adjacent to the mop sink.
    >> Wall paint peeling by three compartment sink.
    >> Holes in wall next to the manager's desk.
    >> Plastic molding around door frame cracked

    >> Water stains on ceiling tiles above the serving line.

    Observation: Employee's personal bags/purse observed stored on the bottom shelf of the preparation table in such a way that it could contaminate food pans.
    Correction: All personal care items, medicines and first aid supplies must be stored in such a way to prevent potential contamination of food, food contact items, equipment, utensils, linens, single-service and single-use articles.

    Observation: Inside surface of the ice maker had accumulation of mold.
    Correction: Clean.

    Observation: The nonfood contact surface of the following equipment has accumulations of grime and debris.
    1. The inside bottom of reach-in number one was soiled.
    2. Hood system dusty
    3. Top of light structures dusty.

    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Observed packages of butter and ranch sauce stored in direct contact with ice.
    Correction: Cease storing packaged food in direct contact with water or ice, unless the contents are draining. The items stored on ice did not need to be stored on ice because refrigeration is not required until after the package is opened.
  • Non-Food Contact Surfaces
    Observation: The following non food contact srufaces need to be cleaned:
    >>Top of oven
    >>Top of hot holding box

    Observation: Plastic cups were found stored on the floor in the dry storage room.
    >>Oven rack stored on the floor in the kitchen.

    Correction: Store plastic cups, and poven racks in a clean, dry location where not exposed to splash, dust or other contamination at least 6 inches above the floor.

    Observation: Bags of trash with food are being stored outside of the trash receptacle on the ground. Dumpster was full at the time of observation.
    Correction: Provide enough pest and rodent proof refuse containers for the storage of all waste with food residue.

    Observation: Dumpster or outside refuse container was open or uncovered.

    Observation: Observed that the floor in the dry storage room is in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Observed that after last pest treatment dead insects are present underneath the shelves in dry storage room.
