Operator claimed chicken wings and fried rice had been cooked at 11 am. Large batch of rice was at 51 F and rice on the counter was 70 F. Only one bowl of chicken wings noted in the fridge so the inspector was unable to identify if the wings were cooling or cold holding. Food should not be cooled on the counter, unless food is in an ice bath.
Operator claimed that facility was under pest control, however could not provide an invoice.
Operator claimed that egg rolls are made every Monday. Egg rolls should be labeled immediately with time.

Cooked chicken (large container) and garlic & oil were observed sitting out at room temperature. These items were also moved to the walk-in.

    Correction: Repair the make table, GE reach-in, and Kenmore reach-in to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Handwashing Lavatory, Water Temperature, and Flow (corrected on site)
    Observation: Water in the building was only reading at 92 F. Water temperature adjusted.
    Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens.
  • Outer Openings - Protected (repeated violation)
    Observation: Screen at back door needs to replaced.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Toilet Tissue - Availability/Toilets and Urinals
    Observation: Toilet tissue was not present at each toilet
    Correction: To minimize hand contact with fecal waste, toilet tissue is necessary for hygienic cleaning following use of toilet facilities. Toilet tissue must be supplied to meet the demand

    Observation: Improper handwashing procedures observed. Employee observed washing hands with latex glove on. Remove gloves prior to hand washing. Ensure hands are washed between changing gloves.
    Correction: Ensure all food employees clean their hands and exposed portions of their arms with a cleaning compound in a lavatory by vigorously rubbing together the surfaces of their lathered hands and warms for at least 20 seconds and thoroughly rinsing with clean water.
  • Food Storage Containers - Identified with Common Name of Food (repeated violation)
    Observation: In use items on cart (such as salt) must be labeled as well as large container of flour.
    Correction: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
  • Utensils - In-Use - Between-Use Storage (corrected on site)
    Observation: Bowls without a handle observed being used to dispense dry goods. Ensure a scoop with a handle is also used for rice.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: Several foods observed sitting on the counter. Foods must be stored at 41 F or below.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: Soap was not provided at the hand washing lavatory in the rest room.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
