    Observation: The raw and/or undercooked foods provided on the menu, menu board, table tent or brochure do not provide a disclosure statement.
    Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods and identification of the animal-derived foods by astericking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients.

Check sanitizer in dishmachine daily using test strips. Kitchen is no longer being used for personal use.
  • Critical: Warewashing - Manual & Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration & Hardness* (corrected on site)
    Observation: Chlorine sanitizing solution used was not at an acceptable concentration in dishmachine-undetectable..
    Correction: Implemented manual sanitizing in 3 compartment sink for all food contact utensils/dishes until machine can be serviced and strength of sanitizer verified.

    Observation: Observed employee cutting lettuce with bare hands
    Correction: Use gloves or tongs or other utensils to pre/handle lettuce and other ready-to-eat foods, such as salad items, sandwiches, fruit toppings
  • Responsibilities of Owner or Proprietor
    Observation: Ash trays observed in screened storage area
    Correction: Remove ash trays or smoking paraphernalia from non-smoking area.
  • Non-Food Contact Surfaces
    Observation: Organic build-up observed on grill/stove
    Correction: Clean outside of stove to prevent harborage conditions for insects and vermin. Correct in 1 week
  • Plumbing System Maintained in Good Repair
    Observation: Mop sink plumbing turned off because of leak. Mop sink was not being used
    Correction: Repair plumbing and use mop sink to dispose of all mop (and other) wastewater. Correct in 30 days
  • Premises - Maintaining Premises - Unnecessary Items and Litter
    Observation: Unnecessary items observed in storage area
    Correction: Remove equipment, foods (from previous owner) etc. that are not essential to the operation of the restaurant. Correct in 30 days
