    Observation: Reach in Refrigerator #2 was observed in a state of disrepair and damaged. RIR not observed maintaining proper ambient air temperatures.
    Correction: Repair the equipment to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the equipment, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils (repeated violation)
    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: Dairy cooler. Dairy cooler was observe with dust accumulations inside the refrigeration unit.
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Conformance with Approved Procedures (repeated violation)
    Observation: Food temperatures are not being maintained for facility's cold holding temperature logs.

Observed food handler cards, employee health policy, and temperature charts. CFM certificate due to expire in May. All PHF/TCS foods observed time and/or temperature abused were voluntarily discarded. Work order observed immediately submitted for RIR #2. Not approved for permit renewal. Instructed CFM to discontinue use of RIR #2 and utilize RIR #1 for all cold holding practices.

Observed invoice for food products delivered this morning at 7:30 AM that were stored in refrigerator #1. All other PHF/TCS foods were voluntarily discarded. Observed temperature charts, employee health policy, and food handler cards.

    Observation: Quiche, milk, and eggs cold holding at improper temperatures due inadequate air flow in refrigerator #1 due to blocked air flow.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
  • Food Labels
    Observation: Foods not properly labelled on dry storage containers.
    Correction: Foods, whether packaged from the provider,packed within the establishment,or contained in working containers are required to conform to specific labeling laws.
  • Handwashing Signage/Handwashing Facilities
    Observation: A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees
    Correction: Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.

Observed food handler cards, employee health policy, and temperature charts. Facility may not provide or use food products from a Farmer's Market. Farmer's Markets are not inspected or permitted facilities and are considered an unapproved source for food products. Approved for permit.

    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.
  • Critical: Preventing Contamination from Hands (corrected on site) (repeated violation)
    Observation: Employees observed handling ready-to-eat (RTE) food with their bare hands. Observed employee bare hand contact RTE sub sandwiches.
    Correction: Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands. Product was voluntarily discarded.

    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: Oven - exterior and superior surfaces. Interior observed clean.
    Correction: Maintain nonfood-contact surfaces of equipment clean.

    Observation: No disposable towels were provided a the hand washing lavatory in the cookline area.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

Observed food temperature charts, employee health policy, and food handler cards. Discussed obtaining food handler cards for teachers who will be participating in serving activities during lunch time. Approved for permit.

    Observation: Clean eating utensils were not observed stored with the food-contact surface facing downward.
    Correction: Store equipment and utensils covered or inverted to prevent contamination while in storage.

Observed food handler cards, health policy, and temperature charts. Facility is anticipating the removal of the automatic closing fire door. All food equipment will be stored on the opposite side of the kitchen and fully covered and protected during the removal construction process. Facility is closing completely for the summer months while students are on summer vacation. All food and nonfood contact surfaces will be properly cleaned and sanitized before closing. Approved for permit.
