Observation: The Person in Charge (PIC) has proof of successful completion of an approved Certified Food Manager (CFM) exam but does not possess the Northern Virginia Certified Food Manager certificate (photo card) issued by ORS Interactive, Inc.
Correction: Obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days to avoid further enforcement action. NOTE: Provide copy of CFM photo ID to Health Department wihtin 10 days.

Observation: The cutting boards along the 2DR PREP COOLER AT THE PIZZA HUT SIDE are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

Observation: The following equipment/utensils were observed soiled to sight and touch: TWO RECTANGULAR PANS ON THE PIZZA HUT SIDE HAD GREASE STAINS.
Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized. PLACED IN WASH BASIN.
- Ice Tea Dispensers & Consumer Self-Serve Utensils Cleaned Every 24 Hours
Observation: Surfaces of the ice tea dispensers and consumer self service utensils are not being cleaned as required.
Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) shall be cleaned at least every 24 hours for iced tea dispensers and consumer self-service utensils such as tongs, scoops, or ladles.

Observation: An air gap between the water supply outlet and the flood rim level AT THE 3 VAT SINK AT THE STARBUCKS SIDE is not observed.
Correction: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch. PLEASE CORRECT WITHIN 10 DAYS FOR REINSPECTION.

Observation: When timed, the (wash, rinse, final rinse) cycle of the mechanical warewashing machine did not meet the manufacturer's guidelines. THE HOBART XLi DISH MACHINE IS NOT CYCLING PROPERLY, AND A WORK ORDER HAS BEEN PLACED. THREE-VAT SINK IS PROPERLY SET UP FOR ALTERNATIVE DISH WASHING METHOD.
Correction: A warewashing machine's conveyor speed or automatic cycle times shall be maintained accurately timed in accordance with manufacturer's specifications. CORRECTIVE ACTION

The purpose of this visit was to conduct a risk factor assessment inspection. This type of inspection focuses on those risk factors that are most likely to contribute to the spread of food-borne illness. Water heater- State sandblaster CSB 8218 SFEX 100 Dish washer- 180-UC The CFM showed copies of the employee health policy that all employees read and sign on a yearly basis. These forms are then sent off to the corporate human resources department to be filed. The EHS has suggested that all food employees have one copy on file at the store for Health Department review.
