The purpose of this visit was to conduct a routine inspection. Quaternary ammonium sanitizer at 3 vat sink: 400 ppm. No violation noted during this evaluation.

Certified Food Manager/Certificate Process Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action. PLEASE EMAIL A COPY OF THE CFM CARD TO THE HEALTH DEPARTMENT WITHIN 10 DAYS.
Handwashing Sink / Used for Hand Wash Only (corrected on site) Observation: Observed the handsink at the PIZZA PREP LINE being used AS A DUMPSINK. OBSERVED FOOD DEBRIS IN THE HANDSINK.
Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. CFM CLEANED THE SINK AND TRAINED THE STAFF.
Removing Dead or Trapped Birds, Insects, Rodents, etc. Observation: Dead COCKROACHES were found AROUND THE HANDSINK AT THE PIZZA PREP LINE AND ON THE FLOOR IN THE BACK ROOM.
Correction: Remove dead rodents, birds, and insects from the premises to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests. DISCUSSED WITH CFM.
Critical: Conditions of Use/Presence & Use of Toxics Per Law (corrected on site) Observation: Pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests. OBSERVED ONE CAN OF RESIDENTIAL PESTICIDE IN THE BACK ROOM.
Correction: Remove residential use pesticides from the facility. When necessary for pest control, chemical pesticides must be applied by a certified pesticide applicator to prevent injury and contamination. CFM DISCARDED THE CAN.

Temp Meas Devices for Cold & Hot Hold Equipment, Present Observation: There was no temperature measuring device located in the following cold or hot holding equipment: True 2 door display cooler, containing items requiring refrigeration.
Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
Refuse Container with Cover/ Women's & Unisex Toilet Room Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall.
Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
Removing Dead or Trapped Birds, Insects, Rodents, etc. (repeated violation) Observation: An accumulation of dead trapped cockroaches was found in the control devices and on the floor near the water heater.
Correction: Remove dead rodents, birds, and insects from the control devices at a frequency necessary to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests.
