    Observation: The handwash station in food prep area is being used for purposes other than washing hands in that said facility observed with a container of chicken noodle soup being thawed in it.
    Correction: The handwash facility identified above is to be used for washing hands only. Operator voluntarily removed said container.

Complaint was received on 6/4/15 stating that three people (working in same company) got sick with diarrhea and vomiting immediately after eating sandwiches from subway. One customer had 6" tuna sub with lettuce, tomatoes and banana peppers on 6/1/15 and started having diarrhea and vomiting. On 6/2/15 other customer a foot long steak and cheese with light mayo and immediately after eating had multiple episodes of diarrhea. Another customer got sick after eating bacon, pepperoni and ham sandwich.
During the inspection all potentially hazardous food were registered at adequate temperature. No bare hand contact were observed. Operator provided records that 226 tuna sandwiches, 166 steak and cheese sandwiches and 245 ham sandwiches were sold between 5/27 to 6/2. As per the operator no employee had called in sick for last one month. The operator was unaware of the complaint and had not received any other complaint. Reviewed employee hygiene and proper handling of food with workers.
The complaint was not found.

    Observation: No disposable towels were provided at the hand washing lavatory in the food prep and service area.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Observation: A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees.
    Correction: Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.

    Observation: The food-contact surfaces of the following equipment were not observed sanitized: The dishes and utensils from the three compartment sink where the sanitizer was measured less than the manufacture concentration level.
    Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.
