There were no signs of pest presence on premises observed at the time of this inspection. Please note the following very important information: ***As mentioned and noted on the previous inspection report, the Owner's name on the business license and on the permit does not match. A previous EHS confirmed with the Virginia State Corporation Commission database that the person named on the business license is the president of the corporation H & K Management Inc. EHS discussed with the manager that the name on the permit should match with the business license. I will provide 10 business days to update the health department with the proper paperwork. MAINTENANCE: *Water Heater: State SBS6565NE Within 10 business days, please fax or email confirmation that the name on the business license has been changed or that the name on the food permit has been changed.
