This was a routine inspection. No critical violations were observed during this visit. Food contact surfaces were noted to be visibly cleaned. There were no signs of pest presence on premises observed at the time of this inspection.
The concentration of the quaternary ammonium compound sanitizer at the 3-compartment sink was checked and noted to be 200 ppm, acceptable.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
  • Refuse / Dumpster / Covers & Lids / Construction
    Observation: The cover of the refuse container located outside the establishment is not a tight fit. The lid is missing.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables used with materials containing food residue and used outside the food establishment shall be designed and constructed to have tight fitting lids, doors, or covers.
  • Lighting Intensity/ Walk-ins & Dry Storage/ 10 foot candles
    Observation: Observed that inadequate lighting was provided in the dry storage areas. Some of the ceiling lights have burned out.
    Correction: Increase the lighting to provide a minimum of 10 foot candles at a distance of 30 inches off of the floor in these areas. Replace the burned out ceiling blights in the dry storage room to provide adequate illumination in these area.

The purpose of today’s visit was to conduct a routine inspection. Thank you for your participation as you input allow me to clarify any questions or concerns that may arise during the inspection. Quaternary ammonium tested at 200ppm, facility test kit. Employee health policy in place. Please note the following very important information:
***A Certified Food Manager shall be present during all operating hours. If a Certified Food Manager cannot be on site during operating hours, it is perfectly acceptable to stop the operation and close your door voluntarily until a Certified Food Manager can be present to monitor the facility operations and employees.
MAINTENANCE:
*Water Heater: State SBS6565NE
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. CFM arrived approximately 25 minutes after the beginning of the inspection.

***As mentioned and noted on the previous inspection report, the Owner's name on the business license and on the permit does not match. A previous EHS confirmed with the Virginia State Corporation Commission database that the person named on the business license is the president of the corporation H & K Management Inc. EHS discussed with the manager that the name on the permit should match with the business license. I will provide 10 business days to update the health department with the proper paperwork.
MAINTENANCE:
*Water Heater: State SBS6565NE
Within 10 business days, please fax or email confirmation that the name on the business license has been changed or that the name on the food permit has been changed.
