  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, only blank forms were noted on premises as such, compliance could not be verified. .
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: No Bare Hand Contact with RTE Foods
    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: fully cooked boneless chicken.
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods.
  • Refuse Container with Cover/ Women's & Unisex Toilet Room
    Observation: There is no covered refuse container for the disposal of feminine napkins in theunisex restroom.
    Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
  • Light Bulbs / Locations where Shielding is Required (repeated violation)
    Observation: Light bulb(s) in the in the general food prep area at the back of the pizza oven not covered by a protective shielding. The shield is missing.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.
  • Lighting Intensity/ Food Prep Area / 50 foot candles
    Observation: Observed that inadequate lighting was provided in the food preparation areas. Some of the bulbs in the ceiling light fixtures are burned out.
    Correction: Increase the lighting to provide a minimum of 50 foot candles in all food preparation areas. Replace the burned out light bulbs to provide adequate illumination.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the wall base tiles are misssing near the restroom entrance as well as in the restaroom.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. Replace the missing wall tiles and base covering.
  • Controlling Pests by Eliminating Harborage Conditions
    Observation: Harborage conditions exist.Small flies noted ion premises.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Eliminate harborage conditions to control and prevent the presence of pests on the premises.
  • Maintaining Premises Free of Litter
    Observation: Observed an accumulation of litter in the storage room and closet next to the office
    Correction: Remove all litter from the premises to allow for regular and effective cleaning and to prevent harborage conditions.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the floor in the storage rooms is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
  • Critical: Storage of Toxics, Not Above Food, Linens, Equip., Single-Svc. (corrected on site)
    Observation: Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Spray bottles of cleaning toxic chemicals were found stored on a food prep cutting board.
    Correction: All poisonous and toxic materials shall be stored below food, equipment, utensils, linens, and single service items to prevent contamination.( chemical bottles were relocated away from the food prep station during the inspection).

  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: The temperature measuring device located in the 3-door flat top prep cooler is broken and not working.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display. Replace the broken thermometer to properly monitor the air temperature inside this unit.
  • Equipment / Non-food Contact / Visibly Clean
    Observation: The nonfood-contact surfaces of the pizza oven were observed soiled with dust and sticky debris.
    Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.
  • Light Bulbs / Locations where Shielding is Required
    Observation: Light bulbs on the ceiling in the dry storage room used for storing pizza boxes are not covered by a protective shielding.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles. Replace the missing ceiling light shield.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the base of the walls inside the restroom is broken and in need of repairs. Also, some of the ceiling tiles are either missing or not properly in place.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.Repair the broken wall base and replace the missing ceiling tiles.

The purpose of todays visit was to conduct a routine inspection. Facility observed in a neat and organized condition. Sanitizer tested at acceptable levels. Food temperatures within acceptable levels. Thank you.

    Observation: Observed that the floor tiles which are observed peeling, chipping and cracked throughout the facility are not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

  • Physical Facilities Good Repair
    Observation: Observed that the following areas are in need of repair:
    1. Cove moulding below the walk in refrigerator
    2 Cove base near bathroom door is separating from wall
    3. Caulking along the three vat sink needed to be smooth and cleanable
    4. Three absorbant ceiling tiles in the dishwashing/dry storage area.

    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
