This visit was made to conduct a pre-opening inspection. All items that were listed on the pre-opening inspection from 8/11/15 were corrected. A 36kw hot water heater was installed as approved during the plan review. Recommend issuance of Alexandria Health Department permit. Ok for C/O. This report will serve as your permit until the official permit arrives in the mail. Any changes to floor plan, equipment, or menu/special processes shall be pre-approved by the Alexandria Health Department. Note: Food Safety Information Binder provided and discussed. "Big 6" Employee Health Poster given. Ensure you have irreversible test strips for checking the high-temperature dish machine. This visit was made to conduct a pre-opening inspection. The following items need to be corrected prior to approval of permit: 1) A 24kw hot water heater with a 98GPH recovery rate was installed when the approved hot water heater was a 36kw with 148GPH recovery rate. Please install the approved hot water heater, or a similar commercial hot water heater that is at least 36kw and provides at minimum 140GPH of hot water. 2) Pipes under the 3-compartment sink are splashing and discharging onto the floor around the floor sink. Pipes shall be installed to where the water does not spill over/outside of the floor sink. 3) Sanitizer and soap dispenser lines were extended below the flood rim of the 3-compartment sink. CORRECTED - Lines were cut above the flood rim of the sinks. 4) Dish machine waste water discharge line is resting inside of the floor sink. Cut the line so that it is above the flood rim of the floor sink. 5) Manger stated dish machine is set up as a high temperature sanitizing dish machine. At time of inspection it failed to register 160F on thermolabel. Ensure dish machine is capable of properly sanitizing dishes (dish surface temperature must reach 160F) prior to final pre-opening inspection. Please call for another pre-opening inspection when the above items have been corrected. Approved for stocking dry goods and frozen foods if approval is given by Ctiy. *Employee health poster and "inspection request" sticker with QR code given to manager.
