This visit was made to conduct a routine food safety evaluation. The Person in Charge was very helpful during the visit. Thank you. The following issues require attention: 1. Please obtain a test kit for the quaternary ammonium used to sanitize in the establishment. Ensure that the concentration of the solution is between 100 - 400 ppm. Do not mix the quaternary ammonium solution with bleach (chlorine) 2. Do not store any insecticides in establishment. Only a licensed pest management control company can apply these chemicals in a food establishment. Note: An updated Employee Health Policy Poster was provided to the Person in Charge and its details were discussed. *Repeat Observations are Subject to Civil Penalty
- Sanitizer Test Kit Required
Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions. There is a test kit for chlorine which is also used in the establishment but no test strips for the quaternary ammonium.
Correction: Obtain a multi quaternary ammonium test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
- Conditions of Use / Presence and Use / Restricted Use Pesticide (corrected on site)
Observation: Observed 3 spray containers of Raid insecticide under the three compartment sink.
Correction: Because of their toxicity, restricted use pesticides shall only be applied by a certified applicator. Spray containers were removed from the establishment.
- Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
Observation: Observed high concentration of quaternary ammonium solution used to sanitize in sanitizing bucket. Concentration was in excess of 400 ppm
Correction: Chemical sanitizers shall be used in accordance with the CFR. High concentrations of sanitizer in excess of the CFR requirements can be harmful because residues of the materials remain. Instructed Person in Charge on how to make proper concentration of sanitizer. Solution was remade to concentration of 300 ppm

This visit was made to conduct a routine inspection. Please note the following items: 1) Provide a quaternary ammonia sanitizer test kit for testing your sanitizer concentration levels in your 3-compartment sink and sanitizer buckets. 2) Ensure that you are air drying your equipment (plastic containers) before you are stacking them together. Observation: Plastic storage containers were found stacked while wet after cleaning and chemical sanitization.
Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food.

- Critical: Food-Contact Surface Sanitized before Use after Cleaning (corrected on site)
Observation: The food-contact surfaces of the following equipment were not observed sanitized: plastic and metal pans. Observed food employee wash and rinse dishes and then place on drainboard for drying. As per certified food manager (CFM) equipment and utensils are not sanitized before use (did not know it was required).
Correction: In conjunction with Section 4-703.11, the food-contact surfaces of equipment and utensils shall be sanitized before use and after cleaning. Corrected by discussing with the CFM the necessity of properly sanitizing food contact surfaces. Sanitizer set up in 3-compartment sink and equipment was properly santized.

- Food Storage / Preventing Contamination
Observation: Food stored on the floor and/or food stored less than 6 inches off the floor. The sugar was improperly stored on the floor.
Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
- Dressing Rooms and Lockers to be Kept Orderly
Observation: Observed employee clothing and other possessions stored in areas that may contaminate food, food-contact surfaces of equipment, single-service items, or single-use items.
Correction: Instruct employees to use the designated lockers and clothes hooks to prevent contamination of food, food equipment and utensils, and preparation surfaces.
- Critical: Restricted Use Pesticides, 40 CFR Criteria (corrected on site)
Observation: Restricted use pesticides do not meet the requirements in 40 CFR 152 Subpart 1 Classification of Pesticides. Raid was improperly stored on the storage shelf with single service and food items. Raid was relocated during the inspection.
Correction: Restricted use pesticides that do not meet the requirements of 40 CFR 152 may be toxic, which may result in the contamination of food. These pesticides shall not be used.
