Observation: Shelf beside back handsink was observed in a state of disrepair and damaged.
Correction: Shelf to be easily cleanable. This was mentioned on your last report and I was told it had been reported. It has been a year. Time to replace.

Correction: Shield or replace light bulb with a coated or shatter-resistant bulb. Manager intends to replace bulbs with shatter resistant bulbs.

- Hand Drying Provision (corrected on site)
Observation: No disposable towels were provided at the hand washing lavatory in the back.
Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

Note: dishmachine gauge indicates machine is operating at correct temperatures. Health Department tape did not turn solid black - Starbuck's tape (temperature dot) after several passes through machine - both tapes turned. Time control used on creamers.
