Observation: The sanitizer was observed at 10 PPM in dish machine.
Correction: Sanitizer levels are too low. Equipment food-contact surfaces and utensils are therefore not adequately sanitized after cleaning due to improper function of the warewashing machine. Discontinue use of the dish machine until it is able to sanitize from 50-200 PPM chlorine.

- Equipment / Good Repair / Operation (repeated violation)
Observation: The dish washing machine is not operating properly and is unable to sanitizer food contact surfaces.
Correction: Repair/adjust the unit so it is capable of sanitzing food contact surfaces. Manually sanitize the utensils until it is fixed.
- Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 50 ppm chlorine solution.
Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine. (Note: Per the CFM this is a low temperature machine). It does not use hot water to sanitize.
- Sanitizing Solution Test Kit Accurate & Used
Observation: The operator is not testing the chemical sanitizing solution to ensure proper concentration of the solution.
Correction: The operator shall be monitoring the concentration of the sanitizing solution using a test kit or other device.
- Certified Food Manager/Certificate Process
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
- Hand Drying Provision / Individual, Disposable Towels (corrected on site)
Observation: Observed that paper towels were not provided at each handwashing sink.
Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing.
- Physical Facilities Good Repair
Observation: Observed that the handle for one restroom door is broken. It does not lock.
Correction: Repair the physical facilities. Fix the door handle.
- Critical: Sanitizers, 21 CFR Criteria for Food Contact
Observation: The concentration of the quaternary ammonia solution was measured at more than 400 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
Correction: Maintain the concentration of quaternary ammonia solution at 150-400 ppm. Verify concentration using the appropriate test kit. Note: Add more water and dilute the solution.

The purpose of today's visit is to conduct a routine inspection. Note: Please fax a copy of invoice/work order for fixing the dishmachine within 10 days to my attention. Fax# 703-385-9568. Facility Data: Water Heater: Lochinvar HSX24-052 Dishmachine: Hobart LXI-30C (currently not in use). Per manager they are not using it because it is not pulling the chemicals. They are manually washing and sanitizing the utensils.

Correction: Repair the dishwashing machine so it is able to wash, rinse and sanitize the food contact surfaces. Fax invoice for fixing the dish machine within 10 days.
