  • Physical Facilities Good Repair (repeated violation)
    Observation: Repair all gaps
    Correction: holes in floors and walls of lower cabinet to provide barrier to rodent access.
  • Critical: Controlling Pests / Methods to Use When Pests Are Found
    Observation: Fresh mouse droppings observed at all lower kitchen cabinets. Methods used have not satifactorily controlled mouse infestation..
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence. NOTE: Provide service invoice and pest control plan for controlling mouse population to Health Dept. within 10 days.

The purpose of today's visit is to conduct a routine inspection.
This facility receives catered food. They do not prepare any food on-site. No breakfast is served. Catered food arrives at 11:30AM from Schooled House Grill (13800 Coppermind Rd. Herndon, VA) and food is served at 12:00PM.

Note to Manager: Please remove the mice droppings and the two dead mice on the sticky traps in the kitchen and clean the area thoroughly with wash, rinse and sanitize. Monitor the same area in the morning for new pest activity. Seal the holes observed in the kitchen cabinets and call pest control service. Fax the pest control service report to the Fairfax County Health Department within 10 calendar days. Fax#703-653-9448

  • Physical Facilities Good Repair
    Observation: Observed the bottom cabinets interior floor boards located below the 2-vat sink and hand washing sink.
    Correction: Repair the holes to avoid access to the kitchen from pest. Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Removing Dead or Trapped Birds, Insects, Rodents, etc.
    Observation: Two dead mice on a sticky trap was observed inside the cabinets on the hand washing sink side in the kitchen. Also, fresh mice droppings was observed along the same area where the two dead mice was found and underneath the 2-vat sink cabinets.
    Correction: Remove dead rodents and the mice droppings from the premises to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests.

The purpose of today's visit is to conduct a routine inspection.
This facility receives catered food. They do not prepare any food on-site. No breakfast is served. Two other CFMs are available as teachers, however, kitchen CFM arrives at 9:00AM.
Catered food arrives at 11:30AM.
EHS observed mice droppings in the cabinets below the 3-compartment sink and hand washing sink. EHS recommends pest control service to be notify.

No violation noted during this evaluation.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
