  • Equipment / Non-food Contact / Visibly Clean
    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: sticky residue on cabinet handles and crumbs inside drawer
    Correction: DISCUSSED WITH EMPLOYEE TO CLEAN NON FOOD CONTACT SURFACES OFTEN TO PREVENT PESTS. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.

* Ensure employee health policy is current and available throughout all hours of operation.
* Encourage having more than 1 certified food manager on staff to ensure adequate coverage throughout all hours of operation.
  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
    Observation: The person in charge could not provide an adequate employee health policy at the time of inspection.
    Correction: Employee health policy (FDA Form 1-B & red folder) was given to person in charge at time of inspection and was reviewed with current food establishment employees. Ensure the employee health policy is current and available at all times of operation.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

  • Equipment / Fixed / Space / Seal
    Observation: The following equipment is not installed in a manner that allows accessibility for cleaning, nor is it installed with closely abutting surfaces to minimize the need for cleaning: 3VAT SINK AND HAND SINK
    Correction: Caulk hand sink and 3 vat sink to adjacent walls. Equipment that is fixed because it is not easily movable shall be installed so that it is: 1) spaced to allow access for cleaning along the sides, behind, and above the equipment, 2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch, and 3) sealed to adjoining equipment or walls if the equipment is exposed to spillage or seepage.
  • Critical: Air Gap Design Standard, Not Less than 1'
    Observation: An air gap between the water supply outlet and the flood rim level of the 3 vat sink is not observed. Airgap provided for sanitizing basin but NO air break for the wash and rinse basins prior to pipe entering the grease trap.
    Correction: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch.
  • Mops in Air-dry Position
    Observation: Observed that there is no mop rack .
    Correction: Install a mop rack at mop sink. Store mops in a position that allows them to air dry without soiling walls, equipment, or supplies.
