Today, follow-up inspection. Establishment received an additional pest control assessment/application yesterday 9/22/14. In addition, entire area was deep cleaned and sanitized. Inspection: Significant reduction in roach presence. However, several juvenile roaches observed in mop sink area--pest control operator notified. Operator is opting to have establishment remained closed through lunch today to ensure that surfaces are again cleaned and sanitized--will reopen at 3:00 p.m. for dinner. Impression: Moderate cockroach infestation. Person-in-charge states pest control company stopped performing monthly treatments X 4 months without his knowledge (pest control service is controlled through landlord). Pest control operator treated establishment on 9/19/14. Due to public health risk, operator has agreed to voluntarily close today. During closure, staff will clean and sanitize all surfaces. EHS to perform a follow-up inspection/assessment on 9/23/14. Recommendation for establishment to reopen or remain closed for business will me made at that time.

Observation: Observed methods are not being used to control pests. Today, observed approximately a hundered live cockroaches in kitchen area. Most contained to multiple glue traps. However, several observed crawling on equipment and across floors. All life cycles observed.
Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.

Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the hamburgers that may be served raw and/or undercooked

Observation: Potentially hazardous food cold holding at improper temperatures in drawer refrigerator unit. Internal product temperatures measured 46-48 F (items cold holding in unit < 4 hours)
Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. At time of inspection, temperature setting was adjusted. Person-in-charge to monitor--will have unit serviced if temperature does not stabalize.

Observation: The waste oil container is stored on absorbent material.
Correction: All refuse containers are to be stored on smooth, nonabsorbent material that is sloped to drain.
