    Observation: Clean a) cart next to stove build up of food particles. b) clean sandwich unit surfaces. Grease build up.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: close up all gaps & holes in the store room and on floors. e.g. holes in store room holding dry goods wall at juncture of floor
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Noted a) floors in kitchen needs to be cleaned, build-up of dirt e.g under 3 compartment sink. b) floors in the walk in needs to be cleaned (smells) c) clean floor of store room old mice droppings on floor.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Noted floors worn in a) kitchen area B) Floor tiles missing next to walk in fridge cannot be cleaned. Tiles missing or loose in various areas . Floor is dirty and needs cleaning. clean floors in whole facility cracks and holes in the floor & wall junctions need to be repaired . Repeat violation C) Clean floor drain in the walk in firdge . fish smelling liquid . Clean and sanitize the floor.
    Correction: Repair or replace floor or floor covering to make it smooth and easily cleanable.
  • Pests - Removing Dead or Trapped Birds - Insects, Rodents, and other Pests
    Observation: Noted dead mice and mice feces on the floor of store room . Remove and clean facility and close up cracks at junctures. and loose floor tiles. . (saw orkin pest control ticket dated 04/25/2015 . Get me a copy of the most recent visit.
