The following foods were on the serving line at temperatures noted: breakfast steak (183°), Salisbury steak (162°F), peas (138°F) and mashed potatoes (135°F). Facility has a new manager. No critical hazards observed during this inspection. Single non-critical hazard to be corrected immediately. Thank you.

    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

Hazards 43 and 45 from the April 9, 2014 inspection have been corrected. No critical hazards were observed during this inspection. Pre-cooked, ready-to-eat foods were heated to the following temperatures before being placed on the serving line or in the warmer: corn dogs (165°F), fish sticks (180°F), corn (184°), and pinto beans (155°F). Par-cooked cheese pizza and cheese sticks coming out of oven were 165°F and 180°F, respectively. Temperature charts are maintained
  • Equipment - Good Repair and Proper Adjustment
    Observation: 1) Meat slicer was observed in a state of disrepair. 2) Lights outside the back entry to the kitchen are not working.
    Correction: 1) Repair the meat slicer to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the meat slicer, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter. 2) Repair light fixtures and/or replace light bulbs as needed.
  • Refuse - Covering Receptacles
    Observation: Observed outside refuse container (Dumpster) without lids. Refuse / Litter on ground around Dumpster as well.
    Correction: Cover all waste containers when not in continuous use. Pick up refuse / litter on the ground around the Dumpster.

There were no previous open hazards to follow-up on. A review of the temperature logs shows inconsistencies in recording food temps. Per the person-in-charge (PIC) temperatures are taken daily as required. EHS and PIC discussed which foods need temperatures recorded and which ones don't. Kitchen is clean

    Observation: Two light bulbs under hood above ovens burnt out.
    Correction: Replace burnt out light bulbs.

    Observation: Single service items stored on the floor of the dry storage room.
    Correction: Store single service items at least 6 inches above the floor.

EHS visited facility in regards to a complaint received on September 23, 2013. That particular complaint could not be validated but the concerns of the complainant were discussed with the entire kitchen staff. The following food items are received raw and must be cooked to proper temperatures before serving: sausage patties, turkeys for the Thanksgiving meal, chicken, and hamburger. The facility also receives pre-cooked hamburgers and pre-cooked chicken products such as chicken nuggets, breakfast chicken and chicken fajita meat. EHS reviewed temperature control logs and verified that temperatures are being taken and recorded when appropriate. The facility does not store any leftover food and does not hold food greater than 4 hours from the time it's cooked or heated. Facility uses time, not temperature, as a control, so there is no set temperature that they must heat pre-cooked food items to before serving.

Inspection focused on cold holding temperatures and food handling. All temperatures good and observed good handwashing and glove use. HACCP sheets kept and up to date. Thank you.
  • Physical Facilities in Good Repair
    Observation: Tear in insulation around air ducts over prep table in kitchen allowing condensate to drip on prep table.
    Correction: Seal tear in insulation to prevent condensate from dripping on prep table.

    Observation: The refuse container located outside the establishment is missing its cover.
    Correction: Replace the missing cover of the outside refuse container with one that is a tight fit.
