Spoke to Owner on site in regards to inspection. Owner paid for two employees to re-test (CFM) via phone during inspection. Follow-up inspection required. Follow-up inspection to take place on 12-21-15.
  • Critical: Person in Charge - Assignment of Responsibility* (repeated violation)
    Observation: The person in charge was not available at the time of inspection.
    Correction: The permit holder shall designate a person in charge who is authorized to monitor and manage all food establishment operations and who is authorized to take actions to ensure that the Regulations' objectives are fulfilled.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (repeated violation)
    Observation: The raw animal food on display is stored in a manner that may cause cross contamination of to ready-to-eat food (RTE).
    Correction: Separate raw animal foods during storage, preparation, holding, and display from raw RTE food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

Spoke to Owner on site in regards to inspection. One employee has registered for October 2015 CFM Class (saw receipt). Permit fee paid over phone during inspection. Recommend renewal of Health Dept. Permit. Permit will be issued when payment has been processed.
  • Critical: Demonstration of Knowledge*
    Observation: The person in charge failed at being a certified food protection manager who has shown proficiency of required information through passing a test that is part of an accredited program or responds correctly to the EHS's questions as they relate to the specific food operation.
    Correction: Ensure that the person in charge has a valid certified food protection manager certificate and/or correctly demonstrates knowledge of the food operation as it relates to the EHS's questions.

    Observation: Employees observed handling ready-to-eat (RTE) food with their bare hands.

    Observation: Handle of back Pepsi Unit was observed in a state of disrepair and damaged (broken).
    Correction: Repair the handle to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the handle, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Both of the handwashing facilities located at the kitchen is blocked, preventing access by employees for easy handwashing.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the plates and other dishes preventing its use.

Spoke to Owner on site in regards to inspection. One employee has registered for Jan 2015 CFM Class (saw receipt). Owner stated that two other employees would register for Jan 2015 CFM Class.

  • Critical: Backflow Prevention, Air Gap* (repeated violation)
    Observation: An air gap between the water supply outlet and the flood rim level at the 3 compartment sink is not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system.
    Correction: Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.

Spoke to Owner on site in regards to inspection. Informed Owner that CFM is needed at facility at all times during operation. Owner stated that one employee is registered for CFM class. Follow-up inspection required. Follow-up inspection to be conducted on 7-16-14. Left FHC Schedule, CFM Schedule, and temperature logs on site.
