    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: No parasite destruction records maintained for fresh raw salmon that are frozen in the restaurant. (Freezer logs discussed. A freezer log form has now been provided to the operator & will be checked at each upcoming routine inspection.)
    Correction: Obtain a written agreement or statement from the supplier stipulating that the fish supplied are frozen throughout to a temperature of -4°F (-20°C) for 168 hours (7 days) in a freezer or at -31°F (-35°C) for 15 hours in a blast freezer.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Floor areas behind and below the deep fryers are noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Unlabeled food containers in the kitchen and at the tea preparation counter. Remember to label all containers with the name of the food.
    Correction: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
  • Cloths - Wiping Cloths - Use Limitation (corrected on site) (repeated violation)
    Observation: Wiping cloths improperly stored between use at a kitchen perp table.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

Clearly label or otherwise mark the food with an easy to follow method, such as use of an erasable marker on the food's container or use a time log ( dry erase board) kept near the sushi rice station.
