The purpose of the visit was to respond to a complaint regarding the Halal meat not being honestly presented. Discussed the meat supplier with the owner. Contacted the meat supplier and confirmed the meat was Halal.

On 3-28-14 a site visit was made regarding the complaint. All staff observed wearing hair restraints & gloves at time of inspection. Discussed safe hygiene practices, hand hygiene practices, and proper changing of gloves with certified food manager and staff. Employees observed adequately washing hands as needed, as well as changing gloves as needed. Ensure employees wear hear restraints, wash hands as needed, and change gloves as needed to prevent the potential of cross contamination. Complaint could not be verified at this time, and therefore complaint will be closed.

* The purpose of today's inspection was to conduct a routine inspection in conjunction with a complaint investigation.

* Ensure employees are trained on reporting symptoms of & diagnosis of communicable disease to prevent the potential of food-borne illness.
* Violations corrected at the time of inspection.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
    Observation: At the time of inspection there was no verification or documentation of adequate employee health policy.
    Correction: Corrected at the time of inspection. Ensure employees are trained on monitoring symptoms associated with communicable diseases, and report any symptoms or diagnosis from a physician to manager. Adequate documentation of training and or education shall be available in the establishment during normal operating hours.
  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Ice scoop and rice scoop observed being stored with handle in direct contact with food/ice.
    Correction: Utensils relocated at the time of inspection by certified food manager. Ensure in-use utensils(scoops for ice & rice) are not being stored with handle in contact with foods or ice.

Email a picture of the modified pre-wash sprayer and a picture of the new plumbing at the 3 vat sink to the Health Department within four(4) days.
**Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding violations are to be corrected within ten (10) days but no later than the time of your first routine inspection (approximately 30 days). Equipment additions/replacement/changes are not allowed without Health Department approval.**
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
Recommendation is made to have exhaust hood cleaned at least every six months. Maintain records of hood cleaning for Health Department review. Hood was cleaned in November 2013.
Recommendation is made to clean hood filters at least every two weeks.
It is recommended that the grease trap be cleaned at least every two months. Maintain records of grease trap cleaning for Health Department review.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.
Water heater: A.O. Smith BT 100 300

  • Critical: Plumbing System / Design, Construct, & Install / Per Law
    Observation: The wash and rinse sinks do not drain through the grease trap. The sanitize sink is directly plumbed to the grease trap.
    Correction: Re-plumb the plumbing at the 3 vat sink. The wash and rinse sinks must drain through the grease trap - there must be an air break before they enter the trap. The sanitize sink must have an air gap before it drains to the floor drain or the grease trap.
  • Critical: Backflow Prevention, Air Gap / When Required
    Observation: Observed the pre-wash spray hose to extend below the flood rim level of the sink basin at the 3-vat sink.
    Correction: Modify or replace the pre-wash sprayer to provide an air gap of approximately 2" between the bottom of the spray head and the top of the sink edge when the pre-wash sprayer is hanging free.
