    Observation: The person in charge failed to state the required temperatures and holding times for the following conditions of potentially hazardous food: (1) Refrigerated storage, (2) Hot holding, (3) Cooling, and (4) various cook temperatures for raw meats.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled. Person in charge should complete an approved food safety class. Please provide copy of approved food safety class registration to the Montgomery Health Department within 10 days.

    Observation: Wiping cloths improperly stored between use.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

    Observation: Shell eggs, pancake batter cold holding at improper temperatures
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.

    Observation: The ready-to-eat (RTE) commercially processed sour cream and garlic in the refrigeration unit was not properly dated for disposition after opening.
    Correction: Mark a "consume by" date on commercially processed RTE foods at the time the original container is opened. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Critical: Ready-to-Eat - Potentially Hazardous Food - Disposition* (corrected on site)
    Observation: The prepared ready-to-eat (RTE) made from scratch dressings, and tomato basil soup, in the refrigeration unit was not discarded by the ""consume by"" date.
    Correction: Discard the food at this time and ensure all prepared RTE food is served, sold or discarded by the ""consume by"" date. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Warewashing Machines, Sanitizer Level Indicator
    Observation: The warewashing machine is not equipped with a device to warn of low sanitizer level.
    Correction: Alter or replace the warewashing machine so that an audible or visual warning is given when more chemical sanitizer needs to be added. Correct within 30 days.
  • Sanitizing Solutions, Testing Devices
    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a quaternary ammonia test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level. Correct within 15 days.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Unused or non-functioning chemical dispensing equipment at the mop sink not removed from the premises. Walk-in freezer door observed not able to close with ice formation around the door. Towel dispenser at the hand sink observed to be inoperable.
    Correction: Remove any unused or non-functioning equipment from the premises. Repair the freezer door so that frost build up does not cause the entire unit to malfunction. Repair the towel dispenser. Correct within 30 days.
  • Critical: Equipment - Food Contact Surfaces and Utensils (corrected on site)
    Observation: Food contact surfaces of the meat slicer used to prepare/store potentially hazardous food items were observed soiled with accumulations of food residues.
    Correction: Clean food-contact surfaces of meat slicer no less than every 4 hours to prevent the growth microorganisms on those surfaces.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the equipment throughout the kitchen has accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents. Correct within 15 days.
  • Handwashing Lavatory, Water Temperature, and Flow
    Observation: Water from the handwashing sink at the kitchen entrance was measured at a temperature less than 100°F.
    Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens. Correct within 10days.
  • Plumbing System Maintained in Good Repair
    Observation: Mop sink observed leaking at the backflow preventer. Sink did not appear to be in continuous use.
    Correction: Plumbing systems and components shall be maintained in good repair. Correct within 15 days.
  • Hand Drying Provision
    Observation: No disposable towels were provided at the hand washing lavatory in the kitchen
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials. Correct within 1 day.
  • Lighting, Intensity
    Observation: Less than 50 foot candles of light was noted in the kitchen area
    Correction: Replace the missing bulbs to provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor. Correct within 30 days.
  • Employee Accommodations, Designated Areas
    Observation: Cell phone observed stored on the shelf above the food preparation area. Other clothing articles observed throughout the kitchen area.
    Correction: Designate an area for employees to eat, drink and use tobacco away from food, equipment and single-service and single-use articles to protect them from contamination. Correct within 1 day.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Floors under the equipment throughout the facility noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner. Correct within 30 days.

    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.
