Note:observed ceiling tiles not properly seated/broken/stained - as ceiling tiles in the back room and in food handling areas are replaced obtain and install washable (vinyl) tiles - do not use perforated ACT.

    Observation: Bug spray present in the facility.

Note: facility walk in cooler not in use - was down and condenser unit and fan coil replaced earlier today. PIC stated that food was brought home - do not use food that was stored at home for customers - if cooler issues are experienced in the future make use of coolers containing non-PHF beverages.

    Observation: Hand washing error - food service worker lathering for less than 10 seconds after applying soap.
    Correction: Instruct all staff on correct hand washing procedures.

    Observation: Employee snack food (open package) stored in close proximity to clean disposable items.
    Correction: Store employee food in the break area.
  • Critical: Toxics - Restricted Use Pesticides - Criteria* (corrected on site)
    Observation: Residential bug spray cans observed in the storage area.
    Correction: Remove the cans from the facility.

ACR 27441
A complaint was received that stated a customer saw roaches in the facility during daylight hours. The PIC was not aware of the complaint. When asked PIC indicated that a new pest control vendor was hired and produced service reports dated 01/28 and 02/13. During the inspection EHS observed five pest monitoring station in the front of the house and all were clean. Two out of five pest control monitoring station in the back of the house showed evidence of roaches. Some additional evidence of pest activity was observed in the cabinet underneath the soda fountain. EHS advised PIC to remove unused items from the facility to facilitate cleaning. Some areas such as walls, cove base and counter top were observed in poor condition and should be scheduled for repair/replacement.
At this time the Health Department had not received any other complaints.

    Observation: The hand sink was observed being used for purposes other than handwashing.
    Correction: Remove the items stored on the sink and maintain access for handwashing at all times.
