    Observation: The following foods were observed cold holding at improper temperatures in the prep unit: cheese 48*F and sauteed onion 46*F. PIC removed PHF to the WIC and turned the small prep cooler's temperature down.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: The door gasket to the chest RIF is in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

    Observation: The cavity of the microwave oven is observed soiled.
    Correction: Using the manufacturer's recommended cleaning procedure, clean the microwave oven at least every 24 hours.

The above observations and the following were discussed with the person in charge:
-No food prep observed during this inspection.
-Employee health policy observed.
-Ice build up observed between the doors of the reach in freezer, PIC stated a work order has been placed to repair the issue.

  • Food Storage - Clean and Dry Location
    Observation: Food stored in a location where it is subject to splash, dust or other contamination. Observed foods stored in the 2 door reach in freezer without being covered.
    Correction: Store food where it is not exposed to splash, dust, or other contamination to prevent contamination.
  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (corrected on site)
    Observation: The ready-to-eat (RTE) commercially processed deli meats and chili in the walk in cooler was not properly dated for disposition after opening.
    Correction: Mark a "consume by" date on commercially processed RTE foods at the time the original container is opened. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The 3 compartment sink and the handsinks in the restrooms are no longer sealed to the adjoining equipment or walls.
    Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.

    Observation: The cutting board(s) along the prep cooler are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

    Observation: Chemical spray bottles stored throughout the facility are not properly labeled. PIC labeled the spray bottles during the inspection.
    Correction: Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.

-Employee health policy observed.
-Seal the sink in the ladies restroom to the wall.
-The quaternary ammonium sanitizer in the 3 compartment sink measured at 200 ppm.
-No food prep observed during this inspection.

    Observation: According to the ""consume by"" date on the prepared ready-to-eat (RTE) foods in the refrigeration unit, the foods should have been discarded. Observed prepared chili, homemade salsa, egg rolls, sliced turkey and sliced ham all stored in the walk in cooler with dates beyond the 7 day limit. Discussed time frames for holding foods with the person in charge.
    Correction: Discard the food at this time and ensure all prepared RTE food is served, sold or discarded by the ""consume by"" date. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The chest freezer and the reach in freezer were observed in a condition that prevents necessary maintenance and easy cleaning. Both freezers observed to have a large accumulation of ice build up.
    Correction: Repair/replace the freezers to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.

    Observation: The nonfood contact surface of the following equipment have accumulations of grime and debris: 1. the hood vents 2. areas of the fryer 3. under the grill plates.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: The handwashing station in the bar is being used as a dump station. Observed food debris in the handsink in the bar.

    Observation: The wall in the men's room is not maintained in good repair. Observed a hole in the wall by the men's room toilet.

    Observation: The floors throughout the bar area noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. The above observations and the following were discussed with the person in charge:
-Please provide Quaternary Ammonium test strips to test the sanitizer used in the 3 compartment sinks. Bleach test strips were provided. The Quaternary Ammonium sanitizer in the 3 compartment sink in the bar was measured at 200 ppm.
-The handsink in the ladies restroom was observed to be cracked and broken along the side of the basin. The legs of the sink also appear to be damaged. Please repair/replace the handsink as needed.
-Please replace the rail attached to the wall along the side of the toilet in the handicapped stall in the men's restroom.
-Employee health policy reviewed.
-Please provide a thermometer in the beer reach in cooler in the bar.

    Observation: Food in contact with equipment or utensils that have not been cleaned or sanitized as required. Per conversation with the person in charge, all in use utensils are being washed, rinsed and sanitized twice a day. The facility is open from 11:30 am until 1:30 am. Observed the PIC handle raw hamburger with gloved hands. He then removed his gloves to obtain more food from the walk in cooler and replaced the same gloves to continue food preparations.
    Correction: Use only cleaned and sanitized utensils or equipment during food preparation. Ensure all in use utensils are being washed, rinsed and sanitized at least once every 4 hours.

    Observation: The prepared ready-to-eat (RTE) foods in the refrigeration units are not properly dated for disposition. Observed several foods in the walk in cooler and prep table that were not dated with either the made on date or discard by date. Several foods had the wrong date on them as well.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The cutting board along the prep table is heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: 1. the interior of the microwave 2. the panini grill.
    Correction: Clean and sanitize these surfaces for food contact.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the following equipment have accumulations of grime and debris: 1. the shelf holding the microwave 2. the hood vents 3. the shelf under the drying rack 4. the ceiling vents in both restrooms.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Refuse - Storage Areas, Rooms, and Receptacles, Capacity and Availability
    Observation: There is no refuse container in the ladies restrooms. Both bathroom stalls are equipped with small, covered trash bins attached to the wall but no trash can was provided.
    Correction: Increase the number of approved refuse containers in the ladies restroom to prevent the accumulation of debris.

    Observation: The floors in the ladies restroom are not maintained in good repair. Observed several broken floor tiles in the ladies restroom.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Physical Facilities - Cleaning Frequency and Restrictions (repeated violation)
    Observation: The following areas are noted in need of cleaning: 1. the wall by the handsink 2. the wall behind the ice machine and drying rack 3. the floors in the restrooms.
