PIC indicates they have not received any customer complaints/ calls of illness during this time frame. Pizza Hut must report to the health department if they receive any calls of illness or any illness by employees as covered in employee health policy. | ACR 31576 Caller indicated a cockroach was baked into pizza. Delivered to home 10/2/13. Site visit-spoke to Manager /PIC Daljit Kaur. She made pizza herself, dough and cheese only. Spoke to caller and asked to have delivery person pick it up-caller declined. Cost was refunded. Pizza Hut has had reqular routine pest control visits, including on 10/2/13. Observed report that indicated NO pest activity. Pizza oven observed clean (without any burned crumbs). This was professionally cleaned on 9/29/13-observed report. No pests observed during this visit, nor past inspections including routine visit on 9/23/13. NOTE: Facility is in a strip mall. Locations of other stores including vacant stores was noted.
